摘要
目的 评价X线立体定向放射治疗对脑转移瘤的疗效。方法 单纯全脑照射 2 0例 (WBI组 ) ,单纯X线立体定向放射治疗 1 9例 (SRT组 ) ,X线立体定向放射治疗加全脑放射治疗 39例 (SRT +WBI组 )。WBI组和SRT +WBI组全脑放疗总剂量均为 30～ 40Gy/ 2～ 4周。SRT组和SRT +WBI组立体定向放射治疗 ,每次剂量为 4.5～ 7.5Gy ,每周 3次 ,总剂量 2 1～ 42Gy。结果 局部控制率、局部复发率和因脑转移所致死亡率 ,WBI组分别为 6 5 .0 %、2 5 .0 %和 5 2 .9% ;SRT组分别为 94.7%、5 .3%和1 6 .7% ;SRT +WBI组分别为 89.7%、0和 8.7%。WBI组与其它 2组比较 ,局部控制率、局部复发率和因脑转移所致死亡率均有显著性差异 (P <0 .0 5 )。结论 X线立体定向放射治疗脑转移瘤 ,在提高局部控制率、降低局部复发率方面优于全脑放疗。
Objective To evaluate the therapeutic effects of X ray stereotactic radiotherapy on brain metastases.Methods Twenty patients with brain metastases were treated with whole brain radiotherapy(group WBI),19 patients were treated with X ray stereotactic radiotherapy(group SRT) and 39 patients were treated with X ray stereotactic radiotherapy plus whole radiotherapy(group SRT+WBI).The entire brain were treated with a dosage of 30～40 Gy in 2～4 weeks for patients of group WBI and group SRT+WBI.In stereotactic radiotherapy,the dosage was 4.5～7.5 Gy,three times a week with the total dose of 21～42 Gy for group SRT and group SRT+WBI.Results The local control rate,recurrent rate and mortality from brain metastases were 65.0% ,25.0% and 52.9% in group WBI,94.7%,5.3% and 16.7% in group SRT,89.7%,0 and 8.7% in group SRT+WBI.All differences between group WBI and other groups were found to be statistically significant( P <0.05).Conclusion X ray stereotactic radiotherapy is better than whole brain radiotherapy for brain metastases in local control and reducing recurrence.
