OVCF经皮椎体后凸成形术后骨水泥渗漏的相关因素分析 被引量:8

摘要 目的对骨质疏松性椎体压缩骨折经皮椎体后凸成形术后骨水泥渗漏的相关因素进行分析。方法选取80例接受经皮椎体后凸成形术治疗的骨质疏松性椎体压缩骨折患者为研究对象。根据术后是否发生骨水泥渗漏分为渗漏组和无渗漏组,对发生骨水泥渗漏的相关因素进行分析。结果 80例共110个椎体行经皮椎体后凸成形术治疗,术后有11个椎体发生骨水泥渗漏。渗漏组和无渗漏组比较,术前椎体前中柱平均高度、术前Cobb角、骨水泥注入量、椎体周壁有无破坏等方面存在统计学差异(P<0.05)。Logistic分析显示,术前椎体前中柱平均高度、术前Cobb角、骨水泥注入量、椎体周壁破坏为骨水泥渗漏重要因素。结论术前椎体前中柱平均高度、术前Cobb角、骨水泥注入量、椎体周壁有无破坏等因素是影响术后骨水泥渗漏的重要原因。 Objective To analyze the correlative factors of bonecement leakage after pereutaneous kyphoplasty in patients with osteoporotie vertebral compression fractures(OVCF). Methods Eighty patients with OVCF who underwent percutaneous kyphoplasty were chosen as subjects. According to the occurrence of postoperative leakage of bone cement, they were divided into leakage and non- leakage groups. The correlative factors of bone cement leakage were analyzed. Results Percutaneous kyphoplasty was carried out on 110 vertebrae in the 80 cases. Bone cement leakage occurred in 11 vertebrae after the operation. There were significant differences in the average height of anterior and middle column, preoperative Cobb angle, the injection volume of bone cement, and damage in the vertebral peripheral wall between the leakage and non-leakage groups before the operation ( P 〈 0.05 ). Logistic analysis indicated that the preoperative average height of anterior and middle column, preoperative Cobb angle, the injection volume of bone cement, and damage in the vertebral peripheral wall were key factors of cement leakage. Conclusion The preoperative average height of anterior and middle column, preoperative Cobb angle, the injection volume of bone cement, and damage in the vertebral peripheral wall are important factors that affect the postoperative cement leakage.
