摘要
目的了解我院老年住院患者的住院和死亡特征,以便为老年疾病防控工作提供依据。方法回顾性调查2005年至2012年我院60岁及以上的住院患者,描述老年住院患者的性别年龄分布,罹患疾病构成及死亡情况。采用非参Kruskal-Wallis秩和检验比较住院天数等非正态分布计量资料的组间差异。采用Pearsonχ2检验比较疾病分类构成及死亡人数等计数资料的组间差异。采用Cochran-Armitage Trend检验病死率的变化趋势。结果从2005年至2012年,我院老年住院患者69503人;其中男性37847人,占54.5%;女性31656人,占45.5%。住院时间中位数为14.0天,平均时间17.1±17.0天。我院老年患者所罹患疾病的主要系统分布为循环系统疾病、肿瘤、呼吸系统疾病和消化系统疾病,四个系统总和构成比为57.8%。死亡病例2611人,病死率为3.8%。从2005-2012年,病死率下降趋势有统计学意义(Z趋势=15.96,P<0.01)。结论我院老年住院患者罹患疾病及死亡原因主要以循环系统和肿瘤为主,应加强防治工作。
Objective To understand the hospitalization and death characters of elderly inpatients in our hospital. Methods A retrospective study was conducted on elderly inpatients above sixty years from 2005 to 20i2, and the age and gender distribution, diseased composition and death situation of the elderly inpatients were described. Kruskal-Wallis test was used to compare the inter-group difference of abnormally distributed measuring materials such as length of stay and so on. Pearson X 2 test was used to compare the inter-group difference of enumeration data such as diseases classification composition and death numbers. Cochran-Armitage Trend analysis was performed to evaluate the variation trend of fatality rate. Results A total of 69503 elderly people were hospitalized from 2005 to 2012, including 37847 males ( 54.5% ) and 31656 females ( 45.5% ). The median of the length of stay in hospital was 14.0 days ( Mean ± Std was [17.1 ± 17.0] days ) . Circulatory system diseases, cancer, respiratory diseases and digestive diseases were the most commonly diseases among elder inpatients, accounting for 57.8% of all diseases. 2611 inpatients were died, with a mortality rate of 3.8%. The mortality rate declined significantly from 2005 to 2012 ( Z trend = 15.96, P 〈0.01 ) . Conclusion The main diseases and death reasons of elderly patients in our hospital were circulatory system disease and cancer, and we should strengthen relevant prevention and treatment work.
