摘要
目的应用德尔菲法探讨COPD患者病情发展“快”“慢”的评价标准。方法在前期流行病学调查、文献研究及第1轮专家问卷的基础上，编制第2轮“慢性阻塞性肺疾病发展快、慢评价标准”专家咨询表，发送给中华医学会呼吸病学分会涉及10个省及直辖市的20位专家进行问卷调查，对答卷进行回收，并采用德尔菲法进行分析。结果专家积极系数为100％，专家权威程度为82．57％，在发展慢评价标准的主要指标中肺功能≤Ⅱ级、病程≥15年满分比均〉60％，变异系数均〈O．3，次要指标中各项满分比均〉60％，变异系数均〈0．3；在发展快评价标准的主要指标中肺功能≥Ⅲ级、病程≤5年满分比均〉60％，变异系数均〈0．3，次要指标中除BMI％21kg／m2外，其余满分比均〉60％，变异系数均〈0．3。结论COPD病情发展“快”“慢”评价标准的框架及条目基本得到专家认可，为之后研究病情发展速度不同的两组患者的相关研究提供有力依据。
Objective To discuss the standard of the development speed of chronic obstructive pulmonary disease by Delphi. Methods To compile to second round consultation form of ＂the standard of development speed of chronic obstructive pulmonary disease＂ which is based on the preview research, documents study and first round experts＇ questionnaires, and to send out to experts of Chinese Medicine Association Respiratory Branch, and then to collect the data to analyze with Delphi Method. Results The expert positive coefficients are 100%, the authority coefficients is 82.57%. For the main index in tile slow development standard, the lung function is lower than level two, the period of the disease is longer and equal than 15 years, all the ratio marks over 60%, and the coefficient of variation is lower than 0.3; the items in the secondary index, all the ratio marks over 60%, and the coefficient of variation is lower than 0.3. For the main index in the fast development standard, the lung function is higher and equal than level three, the period of the disease is shorter and equal than 5 years, all the ratio marks over 60%, and the coefficient of variation is lower than 0.3; the items in the secondary index, all the ratio marks except BMI is lower than 21 that is over 60%, and the coefficient of variation is lower than 0.3. Conclusions The frame of the standard has been accepted by the experts, and to provide the basis for the future research on the different development of the disease.
