摘要
目的 :研究高、低通量透析患者透析中血浆细胞因子水平变化及其与内毒素的关系。方法 :选择采用聚砜膜行高、低通量透析患者各 9例 ,测定其透析开始 (0分钟 )及透析后 6 0、90和 2 40分钟时血浆白介素 1β(IL 1β)、IL 6、肿瘤坏死因子α(TNFα)和内毒素水平。结果 :2组患者透析至 6 0分钟时血浆 IL 1β水平最高 (与 0分钟相比 ,低通量组 P>0 .0 5 ,高通量组 P<0 .0 5 ) ,透析至 2 40分钟时水平最低 (与 0时相比 ,低通量组 P<0 .0 1,高通量组 P>0 .0 5 )。2组患者透析中 TNFα水平都呈上升趋势 ,内毒素水平都呈下降趋势 ,但无显著差异。大部分患者透析中 IL 6水平很低 ,只有 5例患者表现为升高。 2组患者在细胞因子、内毒素水平上并无差异 ;透析中细胞因子水平变化与内毒素水平无相关性。结论 :高通量透析与低通量透析相比 ,并不会增加患者透析中细胞因子产生异常及内毒素污染的可能。
Objective:To investigate the effect of high and lowflux dialysis on patients plasma cytokines and endotoxin levels,as well as the relationship between cytokines and endotoxin.Methods:Nine patients dialysed with highflux polysulphone dialyzers (BLS627) and 9 other patients dialysed with lowflux polysulphone dialyzers (F6) were selected.Plasma levels of interleukine1β(IL1β),IL6,tumor necrosis factorα(TNFα),and endotoxin were assayed at the onset of dialysis,and 60,90 and 240 minutes after dialysis. Results: Plasma IL1β levels were highest at 60 minutes after dialysis (vs. the onset of the dialysis, P >0 05 in lowflux group,and P <0 05 in highflux group),and were lowest at 240 minutes after dialysis(vs.the onset of the dialysis, P <0 01 in lowflux group,and P >0 05 in highflux group).TNFα levels were slightly but not significantly increased in two groups during dialysis.On the contrary,endotoxin levels were decreased slightly but not significantly during dialysis.IL6 levels were too low to be detected in most patients,but it was found elevated in 5 patients.No significant difference was found between two groups concerning intradialysis plasma levels of IL1β,TNFα and endotoxin.Obvious relation did not exist between intradialysis cytokines and endotoxin levels.Conclusions:Highflux dialysis did not increase patients' plasma cytokine and endotoxin levels compared to lowflux dialysis.The changes in intradialysis plasma cytokine levels were not related to that of endotoxin.
