摘要
目的 :探讨十二指肠球粘膜 HP定植与细胞增殖的关系 ,以阐明 HP在粘膜病变中的作用。方法 :检测内镜下诊断的正常球粘膜、球炎、霜斑样病变及溃疡的 HP、炎症和 PCNA。结果 :霜斑处 HP阳性率高于溃疡处 (P<0 .0 5) ;炎症程度与 HP密度正相关 (r=0 .861 3,P<0 .0 5) ;PCNALI与 HP阳性率及炎症程度均不呈平行关系 ;霜斑及溃疡处 PCNALI均高于球炎处 (P<0 .0 5)。结论 :球粘膜炎症程度取决于 HP定植密度 ; 级炎症时隐窝细胞处于“高增殖衰竭”可能导致霜斑样病变 ,也是减少 HP定植的保护机制。
Objective:To elucidate the relationship between cell proliferation and Helicobacter pylori (HP) in duodenal bulbs and to clarify the role of HP in duodenal mucosa damage.Methods:HP,inflammation,and proliferating cell nuclear antigen (PCNA) were detected in subjects endoscopically asssessed as normal duodenum,duodenal bulbitis,‘shimofuri’ lesion,and duodenal ulcer.Results:The positive rate of HP in ‘shimofuri’ lesion was higher than that in duodenal ulcer ( P <0 05);The degree of duodenitis was correlative with the density of HP in duodenal mucosa positively ( r =0 8613, P <0 05);PCNA labeling index (PCNALI) was neither parallel to the positive rate of HP nor with the degree of duodenitis.PCNALI in ‘shimofuri’lesion and ulcer sites was significantly higher in comparison with duodenitis( P <0 05).Conclustion:The severity of mucosal inflammation is determined with degree Ⅲ duodenitis passing into ‘high output failure’ may be the pathogenesis of ‘shimofuri’lesion,also the mechanism of reducing HP colonizing in duodenum.
