摘要
目的：评价颈椎过伸性损伤致急性颈间盘突出的手术治疗效果.方法：收入此类手术治疗患者21例,17例行颈椎前路减压、植自体髂骨、钢板内固定术,4例行颈椎后路椎管扩大成形术,早期手术患者12例.结果：随访4~26个月,除2例患者术前Frankel分型A型无恢复外,其余患者脊髓功能均有不同程度恢复,脊髓功能平均提高1.3级,随访中未见内固定物松动、脱落或断裂等并发症发生,固定节段均获得骨性融合.结论：对于颈椎过伸性损伤致急性颈间盘突出的患者,根据影像学表现,合理选择手术方案和手术时机,脊髓功能可获得较理想的恢复.
Objective To evaluate the clinical results of surgical treatment for the acute cervical disc herniation with extension injury of cervical spinal cord. Method 21 patients who had acute cervical disc herniation with extension injury of cervical spinal cord were trea- ted. 17 patients had the method of anterior decompression, anto - graft and internal fixation, 4 patients had the method of posterior open doors. 12 patients had early operation. Results All patients were followed up for 4 to 26 months, Except the spinal cord function of the 2 Frankel A cases gained no recovery,the spinal cord function of the others improved at different extents,The function of the spinal cord im- proved 1.3 grade after operation according to the Frankle classification. There was no hardware related complications such as loosening, un- tightening and brokenness. Bony fusion was found in all the target levels. Conclusion For the treatment of patients who had acute cervical disc herniation with extention injury of cervical spinal cord,the spinal cord function will be well recovery according to the imagine perform- ance and choosing the right operative time and right operative method.
