摘要
目的观察经尿道钬激光膀胱肿瘤切除术(HoLRBT)和经尿道膀胱肿瘤电切术(TURBT)治疗浅表层膀胱癌的疗效和安全性。方法我院于2011年6月到2013年6月收治行手术治疗的表浅层膀胱癌患者271例,根据治疗方法分为HoLRBT组和TURBT组。分别对2组患者手术时间、术中出血量、膀胱冲洗时间、尿管留置时间、住院时间等术中术后情况、手术并发症和2年累积复发率进行比较分析。结果 2组手术时间比较无统计学意义(P>0.05)。HoLRBT组术中出血量、膀胱冲洗时间、尿管留置时间和住院时间明显低于TURBT组,差异有统计学意义(P<0.05)。HoLRBT组患者术后并发症的发生率明显低于TURBT组,差异有统计学意义(P<0.05)。HoLRBT组和TURBT组患者2年累积复发率分别为8%和16.67%,HoLRBT组复发率明显低于TURBT组,差异有统计学意义(P<0.05)。结论 HoLRBT治疗表浅层膀胱癌疗效显著,安全可靠,可显著降低术后复发率,值得临床推广使用。
Objective To compare the efficacy and safety of transurethral holmium laser resection （HoLRBT） and stand- ard transurethral electroresection （TURBT） in patients with superficial bladder cancer. Methods A total of 251 patients with superficial bladder cancer from 3une 2011 to .lune 2013 were divided into the HoLRBT group and TURBT group. The operative time, the blood loss, bladder irrigation time, catheter indwelling time, hospitalization time, complication and cumulative re- currence rate were analyzed. Results There was no statistical significance between the HoLRBT group and TURBT group （P ~0.05）. The operative time, the blood loss, bladder irrigation time, catheter indwelling time and hospitalization time in the HoLRBT group were significantly lower than those of the TURBT group （P〈0.05）. The incidence rate of complications in the HoLRBT group were significantly lower than that of the TURBT group （P〈O. 05）. The cumulative recurrence rate in the HoLRBT group and TURBT group were 8% and 16. 67%, with significant difference between the two groups （P〈O. 05）. Oonclusiorl HoLRBT is worthy of clinical application for good efficacy and safety and lower recurrence rate in patients with su- perficial bladder cancer.
