准分子激光前弹力层下角膜磨镶术与准分子激光原位角膜磨镶术术后干眼的临床对比研究 被引量:5

摘要 目的探讨准分子激光前弹力层下角膜磨镶术(sub-Browman’s keratomileusis,SBK)与准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)术后不同时间干眼的症状及相关指标的变化。方法回顾性分析2010年3月至2011年12月在我院接受准分子激光手术的近视患者。行SBK者106例(212眼)为SBK组,行LASIK者130例(260眼)为LASIK组,分别观察术前及术后1周、1个月、3个月、6个月干眼症状、角膜荧光素染色、非侵入性泪膜破裂时间、基础泪液分泌量,并进行统计学分析。结果术后1个月和3个月SBK组分别有47眼(22.2%)和25眼(11.8%)出现干眼症状,而LASIK组分别有139眼(53.5%)和92眼(35.4%),两组比较差异均有统计学意义(均为P<0.05)。术后1个月和3个月SBK组分别有39眼(18.4%)和18眼(8.5%)出现角膜着色,而LASIK组分别为125眼(48.1%)和81眼(31.2%),两组比较差异均有统计学意义(均为P<0.05)。术后1个月、3个月SBK组基础泪液分泌量和非侵入性泪膜破裂时间分别为(11.28±1.51)mm、(11.56±1.32)s和(13.57±2.36)mm、(14.39±1.01)s,LASIK组分别为(7.45±1.29)mm、(7.83±2.15)s和(8.15±1.80)mm、(8.57±1.46)s,两组比较差异均有统计学意义(均为P<0.05)。至术后6个月,以上各项指标两组比较差异均无统计学意义(均为P>0.05)。结论 SBK较LASIK治疗近视术后干眼发生率低,恢复快。 Objective To evaluate the symptoms of dry eye and related indexes changes after sub-Browman' s keratomileusis (SBK) and laser in situ keratomileusis (LASIK). Methods The patients underwent keratomileusis in our hospital from March 2010 to December 2011 were retrospectively studied. SBK was done in 106 pa- tients (212 eyes) ,LASIK was done in 130 patients (250 eyes). The dry eye symptom, fluorescein staining, Schirmer I test and non-invasive break-up time at post-operation and postoperative 1 week, 1 month, 2 months, 3 months and 5 months were observed and analyzed. Results The dry eye symptom at postoperative 1 month and 3 months in SBK group appeared in 47 eyes (22.2%)and 25eyes ( ll. 8% ) ,respectively,which in LASIK group were 139 eyes ( 53.5% ) and 92 eyes (35.4%), respectively,there were sta- tistical differences ( all P 〈 0.05 ). The fluorescein staining dots at postoperative 1 month and 3 months in SBK group appeared in 39 eyes( 18.4% ) and 18 eyes (8.5%) ,respectively, which in LASIK group were 125 eyes (48.1% ) and 82 eyes ( 31.2% ), there were statistical differences ( all P 〈 0.05 ). The Schirmer Itest and non-invasive break-up time at postoperative I month and 3 months in SBK group were ( 11. 28 ± 1.51 ) mm, ( 11.55 ± 1.32) s and ( 13.57 + 2.36 ) nun, ( 14.39 ± 1.01 ) s, respectively, which in LASIK group were (7.45 ± 1.29) mm, (7.83 ± 2.15 ) s and ( 8.15 ± 1.80 ) mm, ( 8.57 ± 1.46 ) s, respectively, there were statistical differences ( all P 〈 0.05 ). There was no statistical difference at postoperative 5 months in the above indexes between two groups ( all P 〉 0.05 ). Conclusion The incidence of dry eye in SBK group is lower than that in LASIK group with better recovery.
