摘要
目的 :检测准分子激光角膜原位磨镶术 (LASIK)手术前后泪液可溶性白细胞介素 2受体 (sIL 2R)水平与泪膜稳定性的关系。方法 :采用双抗体夹心酶联免疫吸附法(ELISA)测定 6 0眼LASIK手术前后泪液中sIL 2R的含量 ,同时行泪膜稳定性的检查。结果 :LASIK术前 1wsIL 2R含量为 (16 6 .3± 5 6 .4 )U/ml,术后 1w、2w、3w和 4w含量分别为(46 3.8± 95 .1)U/ml、(44 8.7± 83.2 )U/ml、(435 .6± 96 .4 )U/ml和 (398.7± 10 1.2 )U/ml,LASIK术后与术前比较差异均有显著性 (P <0 .0 1) ;但术后不同时间比较差异无显著性 (P >0 .0 5 )。LASIK术前 1w泪膜稳定性均值为 (2 6 .2 0± 15 .12 )s,术后 1w、2w、3w和 4w分别为 (18.5 6± 15 .2 4 )s、(2 0 .35± 13.6 1)s、(2 2 .83± 13.5 7)s和 (2 5 .10± 14 .2 4 )s,术后 1w、2w、3w与术前比较差异有显著性 (P <0 .0 5 ) ;术后 4w与术前比较差异无显著性 (P >0 .0 5 ) ;但术后 1w与术后 4w比较差异有显著性 (P <0 .0 1)。LASIK术后泪液sIL 2R含量与LASIK术后发生泪膜稳定性变化呈负相关 (r =- 0 .76 5 2 ,P <0 .0 1)。结论 :LASIK术后泪液sIL 2R含量的升高表明术后眼局部的免疫调节机制参与了创伤的修复过程。LASIK术后发生泪膜稳定性变化及眼干症状的出现可能与因手?
Objective: To study the relationship between the level of sIL-2R in tears and te ar film stability in the eyes of pre- and postoperative LASIK patients.Methods: Soluble interleukin-2 levels(sIL-2R) in the tears of 60 eyes before an d after LASIK were studied with a monoclonal antibody-based ELISA 1 week before LASIK and 1 week,2 weeks,3 weeks,and 4 weeks after LASIK. Tear film stability was examined during the same visits. Results: The sIL-2R mean value at 1 week before LASIK was(166.3±56.4)U/ml and mean values at 1 week,2 weeks,3 weeks,and 4 weeks after LASIK were(463.8±95 .1)U/ml,(448.7±83.2)U/ml,(435.6±96.4)U/ml,(398.7±101.2)U/ml,respectively. The sIL-2R levels in tears of post-surgical eyes were significantly different from levels in the preoperative group(P<0.01). However,there w ere no statisti cally significant differences(P>0.05). Compared among the four post-surgical gr oups. Tear film stability at 1 week before LASIK was(26.20±15.12) seconds,and at 1 week,2 weeks,3 weeks,and 4 weeks after LASIK it was(18.56±15.24) seco nds,(20.35±13.61) seconds,(22.83±13.57) seconds,and (25.10±14.24) seconds,re spectively. Tear film stability at the time of the first three visits after s urgery was significantly different from the preoperative level(P <0.05). There w as no significant difference between the pre-surgical level and the level at th e 4th-week visit. There was a negative correlation between tear film sIL-2R con tent and changes in tear film stability after LASIK(r=-0.7652,P< 0.01). Conclusion:The levels of sIL-2R in tears suggested that immunoregulation is inv olved in the healing process. The changes in tear film stability and the onset o f dry eye after LASIK are related to the autoimmunity of the eye.
