摘要
目的:通过分析1例医院获得性肺炎患者的药物治疗过程,探讨临床药师在专科医院开展临床药学的方法。方法:对1例医院获得性肺炎患者的一般病史、用药史、疗效观察、不良反应、用药教育等方面进行详细分析,讨论专科医院治疗医院获得性肺炎的合理方案。结果:临床药师从患者病史及用药采集、观察疗效、观察不良反应、抗感染药物应用、专科药物应用等方面分析,该患者的用药方案较为合理,并将有待改善之处及时告知医护人员。结论:分析疗效、不良反应、专科用药是临床药师开展临床药学监护的基本工作内容;深入研究抗感染用药、专科用药等药物使用是对老年患者医院获得性肺炎实施药学监护的关键。临床药师通过分析现状、临床实践、分析并解决问题的持续改进这一循环方式,可促进临床药学发展。
OBJECTIVE：To investigate the approaches for clinical pharmacist to develop clinical pharmacy by analyzing drug treatment for one patient with hospital acquired pneumonia. METHODS： The case history, medication history, curative efficacy, adverse drug reactions, medication education etc in one patient with hospital acquired pneumonia were analyzed in detail and the reasonable treatment regimen for hospital acquired pneumonia in specialized hospital was explored. RESULTS： Clinical pharmacist＇ s analysis based on patients＇ case history, medication history, curative efficacy, adverse drug reaction and use of anti-infective agents and use of drugs in specialized department showed that the treatment scheme for the patient with hospital acquired pneumonia was reasonable. Clinical pharmacist timely informed the medical staff of the problems remained to be solved. CONCLUSION： The major task for clinical pharmacist to carry out pharmaceutical care is analyzing curative efficacy, adverse drug reactions and use of drugs in specialized department. The key to carry out pharmaceutical care for elderly patients with hospital-acquired pneumonia is to perform in-depth study on the use of anti-infective agents and drug use in specialized department. Clinical pharmacist can promote the development of clinical pharmacy through circular approach of analyzing status quo, clinical practice, analyzing and solving of problems.
