摘要
目的:了解门诊抗菌药物处方情况,促进医院合理用药。方法:抽取中山大学肿瘤防治中心2013年第1季度门诊抗菌药物处方1 364张,以《处方管理办法》、《新编药物学》、《抗菌药物临床应用管理办法》、《中华人民共和国药典·临床用药须知》、《抗菌药物临床应用指导原则》为依据,以《医院处方点评管理规范(试行)》《广东省处方点评实施规范(试行)》为标准,从书写规范性及用药合理性角度,判断不合理处方并进行归类分析。结果:不合理处方1 199张,其中书写不规范和用药不适宜处方分别为1 168张(占97.41%)、31张(占2.59%),前者主要表现为医师无指征延长用药疗程、处方临床诊断书写不全等,后者主要表现为用法与用量不适宜、适应证不适宜、遴选的药品不适宜等。结论:我院门诊抗菌药物处方书写不规范处方比例仍较高,抗菌药物的使用仍存在一定问题,需进一步加强药师审方制度,改善合理用药监督反馈管理环节,进一步提高门诊抗菌药物处方合理率。
OBJECTIVE：To investigate the outpatient prescriptions so as to promote rational use of drugs in hospital. METHODS： A total of 1 364 outpatient prescriptions during the first quarter of 2013 in Sun Yet-Sen University Cancer Center were analyzed with regard to the prescription writing and rationality in antibiotic use in accordance with ＂Prescription Management＂, ＂ Newly Compiled Pharmacology＂, ＂ Administrative Measures for Clinical Use of Antibiotics＂, ＂ Chinese Pharmacopoeia-Clinical Dispensing Information＂, ＂Guiding Principles for Clinical Use of Antibiotics＂, with ＂Hospital Prescription Review Management Practices （trial）＂, ＂Guangdong Provincial Prescription Review Practice （trial）＂ as reference criteria. RESULTS： Of all the prescriptions reviewed, 1 199 were irrational, with 1 168 （97.41%） non-standard in writing and 31 （2.59%） inappropriate in drug use. The non-standard writing manifested as prolonged medication in the absence of indications and incomplete writing of clinical diagnosis in prescription; while the inappropriate drug use manifested as inappropriate in dosage and administration, inappropriate in indications or the choice of drugs etc. CONCLUSION ： The proportion of the outpatient antibiotic prescriptions with non-standard writing was on the high side and the antibiotic use had some problems, therefore, it is necessary to strengthen pharmacists＇ prescription-reviewing system, improve management on the feedback of monitoring on rational use of drugs so as to further improve the proportion of rational outpatient antibiotic prescriptions.
