摘要
目的探讨手术前后PRL型和GH型垂体瘤内分泌激素水平监测的临床意义。方法选择经显微外科手术治疗的75例垂体瘤患者,根据手术切除程度分为手术全切组和手术未全切组,对手术前后血清PRL、GH水平进行检测,并进行1年以上的随访和远期疗效评价。结果 PRL腺瘤及GH腺瘤手术全切组患者术后第1天激素水平明显下降,术后第7和14天时激素水平与术前和术后第1天比较,差异有显著性(P<0.05)。手术未全切组术后第1、7和14天时激素水平无明显下降。手术未全切组患者术后第1、7和14天时激素水平明显高于手术全切组,差异有显著性(P<0.05)。34例GH瘤中有22例血糖异常患者,手术全切组GH瘤患者术后第1天血糖明显下降,术后第7和14天时血糖明显下降,与术前和术后第1天比较,差异有显著性(P<0.05)。手术未全切组术后第1、7和14天时血糖水平无明显下降。手术未全切组GH瘤患者术后第1、7和14天时天时血糖水平明显高于手术全切组,差异有显著性(P<0.05)。结论对于垂体瘤患者行手术治疗显著改善其高激素水平状态,术后监测PRL及GH水平变化可以对PRL型和GH型垂体瘤手术切除程度、肿瘤复发的情况进行有效评估。
[ Objective ] To analyze the endocrine hormones in patients with pituitary adenoma after operation. [ Methods ] 75 cases off pituitary adenoma including 41 cases of prolaetin adenoma and 34 cases of growth hormone adenoma were treated by adenomectomy. The dynamic concentrations of serum PRL and GH were monitored before and after microsurgery. All cases had been followed up for more than one year to evaluate the longterm therapeutic efficacy. [ Results ] The level of hormone in the complete tumor removal group of each type of pituitary adenoma on postoperative ld was significantly lower than that on preoperative day. There was no significant difference of the lev- el of hormone in the uncomplete tumor removal group of each type of pituitary adenoma between postoperative day and preoperative day. The levels of hormone in the complete tumor removal group of each type of pituitary adenoma on postoperative 1, 7, 14 d were significantly lower than those of the uncomplete tumor removal group. There were 3 cases of relapse in the uncomplete tumor removal group of prolactin adenoma and 9 cases of growth hormone adeno- ma. 34 cases GH adenoma in 22 patients with abnormal blood glucose, full set of GH adenoma patients with postop- erative day 1 significantly lower blood sugar, 7, 14 days after the blood sugar significantly decreased, compared with preoperative and postoperative day 1, the difference was statistically significant (P 〈0.05). Surgery cut the whole set of 1, 7, 14 days after the blood sugar level no obvious decline. Cut the whole set of GH tumor surgery patients post- operative 1, 7, 14 days cut whole blood glucose levels were significantly higher than surgery group, the differencewas statistically significant (P 〈0.05). [ Conclusion] The degree of tumor removal plays an role in the change of hor- mone level. PRL and GH concentrations could be the major indicators to evaluate the effect of surgery of pituitary adenoma.
