摘要
目的 采用离体大鼠心肌缺血再灌注心律失常模型 ,观察芬太尼和维拉帕米对缺血再灌注心律失常的影响。方法 将 6 0只SD大鼠随机分为 6组 (每组 10只 ) :对照组 (C组 ) ,10 μg/L维拉帕米组 (V组 ) ,10 μg/L及30 μg/L芬太尼组 (F1及F2 组 ) ,10 μg/L芬太尼 + 10 μg/L维拉帕米组 (F1+V组 ) ,30 μg/L芬太尼 + 10 μg/L维拉帕米组 (F2 +V组 )。离体心脏平衡 2 0min后 ,C组继续灌注K -H液 10min ,阻断冠脉左前降支 10min ,复灌 5min ,记录冠脉流量 (CF)及心电图。其余 5组除平衡后换用含不同药物及浓度的K -H液外 ,余同C组。结果 给予芬太尼或维拉帕米后均能不同程度地减少心律失常的时间 ,延长窦性节律时间 (P <0 .0 5 ,P <0 .0 1) ;芬太尼和维拉帕米合用后 ,减慢心率 (P <0 .0 1) ,增加CF(P <0 .0 5 ) ;其减少心律失常的作用更为显著 (P <0 .0 1) ,并未见室颤。
Objective To study the effect of fentanyl and verapamil on the arrhythmia induced by myocardial ischemia and reperfusion in isolated rat hearts.?Methods Sixty SD rats were randomly divided into 6 groups ( n =10 each). After 20 min of equilibration, the hearts were perfused for 10 min with K-H solution (group C) or with K-H solution containing verapamil 10 μg/L(group V), fentanyl 10 μg/L (group F 1), fentanyl 30 μg/L (group F 2), verapamil 10 μg/L and fentanyl 10 μg/L(group F 1+V), or verapamil 10 μg/L and fentanyl 30 μg/L (group F 2+V), they were then subjected to 10 min of ligation of left anterior descending coronary artery, followed by 5 min of reperfusion, coronary flow ( CF ) and ECG were recorded during the whole experiment.?Results After reperfusion, the duration of arrhythmia was significantly decreased and the duration of normal sinus rhythm was significantly increased by fentanyl or verapamil ( P <0.05, P <0.01). In groups F 1+V and F 2+V, the heart beat was decreased ( P <0.01)and the CF was increased ( P <0.05) significantly. The reduce of arrhythmia is more significant( P <0.01). No ventricular fibrillation was observed.?Conclusion Fentanyl and verapamil synergistically attenuate the arrhythmia induced by ischemia and reperfusion in isolated rat hearts.
