摘要
目的 探索出一套适合医院-社区-服务站高血压信息网络化管理体系,并分析其在提高预防、控制和治疗的管理质量,降低高血压患者的发病率及病死率中的作用.方法 通过计算机技术和互联网技术,把医院-社区-服务站连接起来,建立一个强大的高血压管理网络和患者资源库.将2010年1月～2013年6月江阴市第五人民医院（以下简称“我院”）收治的500例高血压患者纳入我院研制的高血压网络化管理体系中.在该体系中,患者基本资料、病史、临床表现、各种检查、并发症等情况,以及饮食、运动、药物治疗方案、后续就诊、随访情况等各项参数均被及时记录.观察并比较应用计算机系统前后患者规律服药率、高血压防治知识知晓率、治疗有效率、血压控制率.结果 计算机管理的应用后患者的规律服药率、高血压防治知识知晓率、治疗有效率、血压控制率（92％、94％、89％、91％）均较应用前明显提高（63％、77％、77％、66％）,差异有统计学意义（P＜0.05）.结论 把先进的计算机技术和高血压管理有机结合起来,利用计算机管理系统对高血压患者进行管理,并为每位高血压患者建立一套“高血压健康档案”,可为我国高血压网络化、信息化、规范化管理探索一条新思路.
Objective To explore a set of service network management system for high blood pressure,which is suitable for hospital-community-service station,and analyze the effect in the improvement of management quality of prevention,control and treatment,and reducing of the incidence and mortality of hypertension.Methods The hospital-community-service was connected through the computer technology and internet technology,formed to be a powerful high blood pressure management network and database.500 cases of hypertension patients who were admitted in the Fifth People＇s Hospital of Jiangyin City （＂our hospital＂ for short） from January 2010 to June 2013 were included in the computer management system,whose basic information,medical history,clinical manifestation,examination,complications,as well as diet,exercise,drugs treatment and some other information were recorded in the system.Regular medication rate,high blood pressure prevention and control knowledge awareness,treatment effectiveness,blood pressure control of all the objects before and after the application of computer system were compared.Results The regular medication rate,high blood pressure prevention and control knowledge awareness,treatment effectiveness,blood pressure control rates increased after the application of computer system （92%,94%,89%,91%） were significantly higher than those before the application of computer system （63%,77%,77%,66%）,the differences were statistically significant （P 〈 0.05）.Conclusion Taking the advantages of computer technology and combinating with hypertension management,using the computer management system to manage the hypertension patients and establish a set of ＂high blood pressure and health archives＂ for each high blood pressure patient can provide a new way of networking,informatization and standardization management for hypertension in our country.
