摘要
在对宁夏基本药物实施现状分析的基础上,从统一招标、统一配送和统一价格3个方面对宁夏药品"三统一"政策实施效果进行了讨论分析。结果认为:统一招标切断了流通领域的药品加成,降低了药品价格,规范了医生行为;统一配送规范了药品流通市场;统一价格使医疗机构规范执行。存在的主要问题有:统一招标透明度不够,药品目录太窄,中标企业履约率低;统一配送率和药品配送数量较低,耽误临床用药和急救用药;配送时间较长,配送数量不全;统一价格太死板,居民用药可及性差。建议取消"三统一"中的"统一配送",改"三统一"为"两统一"。加强招标管理,改进招标体制;制定弹性价格,相对统一;对基本药物实行国家计划生产与管理。
To give policy assessment of essential medicine＂Trinity＂（unified Bidding, Distribution and Price）based on the current essential medicine implement situation analysis in Ningxia. Main findings： the unified bidding policy cut the drug additional profit in circulation and the medical misbehavior was regulated; the unified distribution policy gives regulation on the drug market; and unified price makes medical facilities work in certain regulation. Main existed issues are transparency, narrowed drug list and abidance agreement of unified bidding; lower distribution rate and volume of unified distribution; inflexible price and poor accessibility of unified price. Lastly, it is suggested that unified distribution should be cancelled to ＂two＂ unified policies. Meanwhile, essential medicine bidding should be strengthened with the management mechanism; drug price should be flexible with planned production and management by the central government.
