摘要
目的:探讨靶向注药联合小针刀治疗第三腰椎横突综合征的临床疗效。方法:采用随机对照研究方式,将125例第三腰椎横突综合征患者分为针刀组、注药组和联合组。针刀组单用小针刀治疗40例。注药组在C臂机透视下定向靶点注射混合液(罗哌卡因针10mg、得宝松针7mg、甲钴胺针1000μg)治疗40例。联合组在C臂机透视下定向靶点注射混合液(罗哌卡因针10mg、得宝松针7mg、甲钴胺针1000μg)准确消除病灶的无菌性炎症,再用小针刀治疗45例。结果:联合组与针刀组、注药组治疗后VAS评分、Denis疼痛分级、Oswestry功能障碍指数与治疗前比较均有下降,差异有统计学意义(P<0.05)。联合组能更有效的降低腰部疼痛程度及改善第三腰椎横突综合征的症状。在疗效方面:联合组总有效率为97.8%,针刀组为95.0%,注药组为92.5%。联合组与针刀组、注药组比较,差异具有统计学意义(P<0.05),联合组疗效优于针刀组和注药组。针刀组与注药组比较,差异无统计学意义(P>0.05)。结论:靶向注药联合小针刀治疗第三腰椎横突综合征疗效显著,值得临床推广应用。
Objective：To discuss the curative effect of targeted drug injection combined with small needle knife on the treat- ment of the third lumbar vertebra transverse process syndrome. Metbods..Via randomized comparative controlled study, 125 cases of the third lumbar vertebra transverse process syndrome were divided into small needle-knife group, drug injection group and combination group. Needle-knife group with small needle knife therapy singly included 40 cases. Injection mix- ture （ropivacaine injection 10 rag, Diprospan injection 7rag, Mecobalamin injection 1000μg） was injected to the target by means of C-arm X-ray machine perspective oriented in 40 cases in drug injection group. Also the same style of injection plus small needle knife therapy was adopted in the combination group in 45 cases, which accurately eliminated lesions of aseptic inflammation . Results：The VAS score, Denis pain scale, Oswestry disability index compared with themselves before the treatment were decreased in all group, the difference was statistically significant （P〈0.05）. The combination group can effectively release the waist pain and the symptoms of the third lumbar transverse process syndrome. In terms of efficacy： the total effective rate was 97.8% in combination group, needle-knife group was 95.0%, injection group was 92.5%. The combination group is better than the acupuncture group and drug injection group, and the difference is statistically signifi- cant （P〈0.05） ,. There is no significant difference between needle knife group and drug injection group （P〈0.05）. Con- clusion：Targeted drug injection combined with small needle knife is effective and worthy of clinical application in the treat- ment of the third lumbar transverse process syndrome.
