摘要
目的:观察小针刀松解术配合二仙汤治疗阳虚血瘀型膝骨性关节炎的临床效果。方法:应用随机分组法将90例阳虚血瘀型膝骨性关节炎患者分成治疗组和对照组,各45例。对照组单纯给予患膝关节小针刀松解术,治疗组给予小针刀松解术配合二仙汤加减治疗,2周为1个疗程。1个疗程后观察两组临床疗效,并评价治疗前后两组患者WOMAC评分、TNF-α和NO水平来评价两组患者膝关节功能的改善情况。结果:(1)治疗组WOMAC评分为(13.47±5.65)分,显著低于对照组(18.80±4.88)分,差异有统计学意义(P<0.05);(2)治疗后治疗组TNF-α和NO水平分别为(1.29±0.41)分、(23.68±19.17)分,均显著低于对照组(1.81±0.66)分、(61.77±21.09)分,差异均有统计学意义(PTNF-α<0.05,PNO<0.05)。结论:小针刀松解术配合二仙汤治疗阳虚血瘀型膝骨性关节炎效果显著,能明显减轻患者疼痛,提高患者膝关节功能活动度。
Objective:To observe the clinical effect of small needle knife release combined with Erxian decoction in treating knee osteoarthritis of Yang deficiency and blood stasis.Methods:90 patients with knee osteoarthritis caused by Yang deficiency and blood stasis were randomly divided into treatment group and control group from January 2019 to March 2020 in the outpatient department and inpatient department of affiliated hospital of shanxi university of traditional Chinese medicine.The control group was treated with small needle knife release of the affected knee,while the treatment group was treated with small needle knife release combined with Erxian decoction treatment for 2 weeks.After 1 course of treatment,the clinical efficacy of the two groups was observed.WOMAC score,TNF-αand NO level were evaluated the improvement of knee function in the two groups.Results:The WOMAC score of the treatment group was(13.47±5.65),significantly lower than that of the control group(18.80±4.88).The difference between the two groups was statistically significant(t=2.689,P<0.05).After treatment,the levels of TNF-αand NO in the treatment group were(1.29±0.41)and(23.68±19.17),respectively,which were significantly lower than control group(1.81±0.66)and(61.77±21.09).The differences between the two groups were statistically significant(tTNF-α=2.05,PTNF-α<0.05;tNO=2.11,PNO<0.05).Conclusion:Small needle knife release combined with Erxian decoction is effective in treating knee osteoarthritis with Yang deficiency and blood stasis.
