摘要
目的：探讨高血压病中医不同证型患者心脏结构及左室收缩、舒张功能的改变,分析不同证型高血压病患者的心脏损害程度.方法：选择2011年11月～2012年12月在深圳市中医院诊治的原发性高血压患者100例.将患者分为肝火亢盛、阴虚阳亢、痰湿壅盛、阴阳两虚4型.通过超声心动图测量各个证型患者心脏结构指标及功能指标.运用SPSS 17.0软件进行统计学处理.结果：①心脏结构异常比例：肝火亢盛型＜阴虚阳亢型＜痰湿壅盛型＜阴阳两虚型.②左室收缩、舒张功能减退：肝火亢盛型＜阴虚阳亢型＜痰湿壅盛型＜阴阳两虚型,其中阴虚阳亢型患者左室壁运动幅度增强,左室收缩功能亢进,与其他3组比较差异有统计学意义（P＜0.05）.结论：患者的心脏结构、左室收缩舒张功能可以作为高血压病中医辨证的客观化指标.
Objective： To observe the relationship of the cardiac structures,left vebtricular diastolic and systolic function in pri mary hypertention. To analyze the different heart damage in different types of syndrome. Methods：Totally 100 outpatients or inpatients with essential hypertension in the Shenzhen traditional Chinese medicine hospital between November 2011 and December 2012 were voluntarily involved in the study. The patients were divided into 4 types：live〉fire hyperactivity, hyperactivity of yang due to yin deficiency,excessive accumulation of phlegm-dampness,deficiency of both yin and yang. The cardiac structures and function measurements of the patients were studied by echocardiography. These measurements were analyzed with SPSS 17.0 statistical software. Results：（1）The abnormal cardiac structures：live〉fire hyperactivity group〈（hyperactivity of yang due to yin deficiency group〈 excessive accumulation of phlegm-dampness group〈 deficiency of both yin and yang group. （2）The decreased of left ventricular diastolic and systolic function： liver-fire hyperactivity group〈hyperactivity of yang due to yin deficiency group〈 excessive accumulation of phlegm-dampness group〈deficiency of both yin and yang group. The left ventricular wall and interventricular septum systolic moving amplitude were significantly increased in hyperactivity of yang due to yin deficiency group than other groups（P〈0.05）. Conclusion：The cardiac structures,left ventricular diastolic and systolic function of patients can be the objective indicator when diagnose by syndrome differentiation in traditional Chinese medicine.
