摘要
目的:评价术后早期胸腔热灌注联合全身静脉化疗治疗恶性胸膜间皮瘤的疗效。方法:回顾性分析河南省人民医院胸外科2008年8月至2010年8月收治的10例恶性胸膜间皮瘤患者的临床资料,其中局限型6例,弥漫型4例。2例行探查活检术,8例行手术切除(根治性手术6例,姑息性手术2例)。手术后早期进行胸腔热灌注治疗,术后按培美曲塞联合顺铂方案化疗。结果:3例因肿瘤广泛侵袭及转移化疗无效,于术后3个月内死亡。7例术后存活大于3个月,其中3例复发,复发时间分别为术后第6个月、第13个月、第15个月。术后第6个月、第13个月复发患者未行再次手术治疗,于化疗后2个月死亡。术后第15个月复发患者行再次手术治疗,于化疗后6个月死亡。其余4例长期存活。结论:术后早期胸腔热灌注联合全身静脉化疗是治疗恶性胸膜间皮瘤有效的治疗方法。
Objective ： To evaluate the curative effect of intrapleural hyperthermic perfusion in early stage of post-operation combined with general venous chemotherapy for malignant pleural mesotheli- oma. Methods： 10 cases of malignant pleural mesothelioma hospitalized from August 2008 to Au- gust 2010 were studied by the retrospective analysis. In all the 10 cases, 6 cases were localized and 4 cases were diffused. 2 cases received biopsy, 8 cases received surgery （6 cases received radical operation and 2 cases received palliative operation）. All the patients received intrapleural hyperthermic perfusion in early stage of post-operation and general venous chemotherapy （peme- trexed and cisplatin）. Results ： 3 patients died within 3 months after operation for the extensive in- vasion and distant metastasis of tumor. 7 patients survived for over 3 months. In the 7 patients, 3 patients recurred in 6 months, 13 months and 15 months after operation. The patients recurred in the 6th month and the 13th month didn＇t receive reoperation, and died after 2 months for ineffec- tive chemotherapy. The another one received reoperation and died in the 6th month after general venous chemotherapy. The other 4 patients are long-term surviving. Conclusion： The treatment of intrapleural hyperthermic perfusion in early stage of post-operation combined with general venous chemotherapy is effective for malignant pleural mesothelioma.
