子宫颈癌和CIN患者血浆APC和RASSFlA基因启动子甲基化检测的意义 被引量:10

Quantitative detection of APC/RASSF1A promoter methylation in the plasma of patients with cervical diseases

摘要 目的探讨子宫颈癌和子宫颈上皮内癌变(CIN)患者血浆APC和RASSF、1A基因启动子甲基化水平检测的意义。方法收集2009年5月至2011年6月在南京医科大学第一附属医院就诊的118例子宫颈癌和CIN患者[包括CINI34例(低级别组)、CIN11和CINm43例(高级别组)、子宫颈癌4l例(宫颈癌组)]以及25例同期健康妇女(对照组)的血浆,其中31例患者(包括低级别组6例、高级别组10例和宫颈癌组10例、对照组5例)同时收集其子宫颈组织标本。采用白行建立的双重实时荧光定量甲基化特异性PCR(qMSP)技术检测各组患者血浆APC和RASSFlA基因启动子甲基化水平,评价其对子宫颈癌和CIN的诊断价值,并分析其临床意义。结果31例同时检测血浆与组织标本的患者中,血浆与组织对应标本中APC基因甲基化阳性符合率为3/5,阴性符合率为100%(26/26);RASSFlA基因甲基化阳性符合率为7/9,阴性符合率为100%(22/22)。低级别组和对照组患者(合并后共59例)中APC和(或)RASSFlA基因甲基化阳性率为3%(2/59),宫颈癌组和高级别组患者(合并后共84例)为48%(40/84),两者比较,差异有统计学意义(P〈0.01)。APC和RASSFlA基因甲基化联合检测的敏感度为47.6%,特异度为96.6%。APC基因甲基化阳性率与子宫颈癌患者的年龄、HPV感染型别、临床分期、病理类型、淋巴结转移状态无关(P〉0.05),而RASSFlA基因甲基化阳性率与子宫颈癌患者的淋巴结转移和临床分期相关(P〈0.05)。结论血浆APC和RASSFlA基因启动子甲基化检测在子宫颈癌和CIN的诊断以及衡量疾病严重程度中具有一定的应用前景。 Objective To quantitatively detect adenomatous polyposis coli (APC) and Ras association domain family 1 A( RASSF1 A) promoter methylation levels in the plasma of patients with cervical disease and to determine the diagnostic value of the indicators of cervical disease. Methods Preoperative blood samples were collected from 25 cases of healthy women and 118 cases of cervical disease, and tissue samples were also collected from 31 cases of them. The APC/RASSF1A promoter methylation levels of plasma and tissue were determined by duplex real-time quantitative methylation specific PCR (qMSP) . Results Among 31 paired plasma and tissue samples, true negative rate of APC and RASSF1A genes were all 100% , and true positive rate of APC and RASSFIA genes were 3/5 and 7/9, respectively. In 143 cases of plasma samples, total positive rate of APC and (or) RASSF1A methylation was 3% (2/59)for control/ low-grade lesions groups and 48% (40/84) for high-grade lesions/tumor groups ( P 〈 0. 01 ). RASSF1A methylation rate was related to lymph node metastasis and International Federation of Gynecology and Obstetrics(FIGO) stage( P 〈 0. 05 ). Conclusion The plasma APC/RASSF1 A methylation detection may be with some application prospect in the diagnosis of cervical diseases.
