摘要
目的了解临床分离的屎肠球菌与粪肠球菌的分布及耐药性,为临床治疗提供依据,以提高疗效。方法用WalkAway自动化微生物仪PC20检测板进行菌株鉴定和药敏试验,应用Whonet 5.6软件统计粪肠球菌、屎肠球菌的分布和耐药率。结果 101株屎肠球菌对万古霉素、利奈唑胺、呋喃妥因、氯霉素、利福平、链霉素(高浓度)、庆大霉素(高浓度)、四环素、左氧氟沙星、红霉素、环丙沙星、氨苄西林和青霉素的耐药率分别为0、1.0%、28.7%、46.5%、66.3%、75.2%、79.2%、85.0%、92.1%、94.1%、96.0%、96.0%和97.2%;81株粪肠球菌对万古霉素、利奈唑胺、氨苄西林、青霉素、呋喃妥因、左氧氟沙星、环丙沙星、利福平、氯霉素、链霉素(高)、庆大霉素(高)、四环素和红霉素的耐药率分别为1.2%、1.2%、12.3%、13.6%、14.8%、37.0%、40.7%、52.0%、53.1%、59.3%、60.5%、80.2%和87.7%;屎肠球菌(47.5%)和粪肠球菌(31.7%)主要来源于尿液,其次来源于分泌物和痰液。结论临床已出现耐万古霉素和利奈唑胺的肠球菌,屎肠球菌对常用抗菌药物的耐药率显著高于粪肠球菌,主要引起泌尿系感染。
OBJECTIVE To understand the drug resistance and distribution of Enterococcus faecium and Enterococ-cus faecalis which were isolated clinically, and to provide the basis for clinical treatment so as to improve the efficacy. METHODS Strain identification and drug susceptibility tests were performed using PC20 detection board of WalkAway automatic microbial instrument, and the drug resistance rate and distribution of E. faecium and E. faecalis were counted with Whonet 5.6 software. RESULTS The rate of drug resistance of 101 strains of E. faeci-um to vancomycin, rina thiazole amine, nitrofurantoin, chloromycetin, rifabutin, streptomycin (high concentra- tion), gentamycin (high concentration), tetracycline, levofloxaein, erythromycin, ciprofloxacin, ampicillin and penicillin, were 0, 1.00%, 28.7%, 46.5X, 66.3%, 75.2%, 79.2%, 85%, 92.1%, 94.1%, 96%, 96% and 97.2%, respectively. The rate of drug resistance of 81 strains of E. faecalis to vancomycin coccus, rina thiazole amine, ampieillin, penicillin, nitrofurantoin, levofloxacin, ciprofloxacin, rifabutin, chloromycetin, streptomycin (high concentration), gentamycin (high concentration), tetracycline and erythromycin were 1. 2%, 1. 2%, 12.3%, 13.6%, 14.8%, 37%, 40.7%, 52%, 53.1%, 59.3%, 60.5%, 80.2% and 87.7%, respectively. The E. faecium (47.5%) and E. faeca/is (31.7%) mainly derived from urine, secondly from secretion and sputum. CONCLUSION The vancomycin and rina thiazole amine resistant Enterococcus has appeared clinically. For the rate of drug resistance of common antimicrobial drugs, E. faecium is significantly higher than E. faecalis, which can mainly cause the urinary tract infection.
