气管支架置入术在老年重症气管恶性狭窄患者治疗中的应用 被引量:8

摘要 目的探讨应用气管支架置入术治疗老年重症气管恶性狭窄患者的疗效。方法回顾性分析20例重症气管狭窄的老年患者,其中男性13例,女性7例,年龄60~80岁,平均(68.5±1.7)岁。按照气管狭窄原因分为肺癌9例、食管癌6例、纵隔肿瘤2例、气管肿瘤2例、甲状腺肿瘤1例。所有患者均在气管局麻和数字减影血管造影机透视下置入气管支架。结果共置入气管支架20枚,其中z型支架3枚,镍钛记忆合金支架17枚(其中Y型支架1枚),技术成功率i00%。术后患者呼吸困难即刻缓解,气促评级由Ⅲ~Ⅳ级降至0~I级;血氧饱和度由高流量吸氧时(71.4土2.8)%(范围70%~80%)改善为自然状态下(范围93.2±2.8)%(范围90%~95%)。随访3个月至i年,所有患者在随访期内无再发呼吸困难。结论对于重症气管狭窄的老年患者,气管支架置人术是一种迅速、有效的急救措施,有良好的缓解症状、改善生活质量的作用。 Objective To evaluate the efficacy of tracheobronchial stent placement on elderly patients with severe malignant tracheal stenosis. Methods 20 elderly patients with severe malignant tracheal stenosis (13 males and 7 females, aged 60-80 years, mean (68.5 q-1.7) years3 were retrospectively analyzed. Patients were divided into lung cancer patients (n = 9), esophageal cancer patients (n--6), mediastinal tumor patients (n 2), trachea tumor patients (n= 2), thyroid neoplasm patients (n= 1) according to the cause of tracheal stenosis. The stents were placed in all patients under digital subtraction angiography (DSA) positioning and local anaesthesia. Results 20 patients were treated with 3 Z type stainless steel stents and 17 Ni Ti memory alloy stents (one Y-shaped stent), and the technical success rate was 100%. Hypoxia symptom was immediately relieved in all patients after stent placement. Dyspnea score was improved significantly from Ill-IV to 0- I ,and blood oxygen saturation (SPOz) was elevated from 70%-80% (71.4 ± 2.8)% in high flow oxygen treatment to 90%-95% ((93.2±2.8)%]under normal conditions. No dyspnea was found in patients during the 3-month to 1 year follow-up. Conclusions Tracheobronchial stent placement is a safe and rapid emergency treatment for symptoms relief and improving the quality of life in elderly patients with severe malignant tracheal stenosis.
