摘要
目的 :探讨急性白血病 MDR1 + / CD34+表型的临床意义。方法 :采用流式细胞仪及对碘硝基四唑盐(INT)细胞毒检测法 ,分析了 5 1例初治及 17例复发急性白血病患者细胞的表面分化抗原及 MDR1 + 基因编码的p170阳性率 ,并用 INT法的体外药敏试验进行对照。结果 :MDR1 + 与 CD34+ 有显著相关性 (r =0 .842 ,P <0 .0 1) ,而与 CD1 3、CD1 4 、CD1 5 、CD33、HL A- DR等其它髓系抗原无关 ;MDR1 + / CD34+表型细胞较 MDR1 - / CD34-表型细胞缓解率、生存期、缓解期显著缩短。结论 :急性白血病 MDR1 + / CD34+ 表型可作为白血病预后不良的指标。
Objective:To detect the expression of the multidrug resistance(MDR) gene product(P-glycoprotein) and differential antigen, clinical significance of MDR 1 +/CD 34 + phenotype in the patients with acute leukemia.Method:By means of flow cytometry(FCM) and P-iodonitrtetrazolium violet(INT) method, 51 cases and 17 cases were studied including,previously untreated group and relapse group,respectively.Result:The results showed the MDR 1 + phenotype correlated with CD 34 + phenoype in acute leukemia(r= 0.842 ,P< 0.01 );In the resistance drug group,the ratio of the expression of CD 34 + and MDR 1 + was higher than that of sensitivity group(P< 0.01 ,P< 0.05 ),There were no correlations among MDR 1 + and CD 13 、CD 14 、CD 15 、CD 33 HLA-DR;The ratio of CR and the time of disease free surival (DFS) with the MDR 1 +/CD 34 + phenotgpe was significantly less than those of the MDR 1 -/CD 34 - phenotype.Conclusion:The MDR 1 +/CD 34 + phenotype is a poor factor in the prognosis of patients with acute leukemia.
