经胸与经食管超声心动图在二尖瓣脱垂亚区分区中的应用研究 被引量:3

摘要 目的应用经胸超声心动图及经食管超声心动图技术对比研究二尖瓣脱垂六叶亚区分区,并与外科术中结果进行比较,从而评价两种检查方法的诊断价值并初步探讨二尖瓣脱垂不同亚区与二尖瓣反流之间的相互关系。方法选择二尖瓣脱垂伴有中度以上二尖瓣反流患者33例。所有研究对象首先经胸超声心动图技术初步确定二尖瓣脱垂,然后应用经食管超声心动图技术进一步确定二尖瓣脱垂的亚区分叶,再分别与外科术中结果进行比较。二尖瓣亚区解剖定位采用Carpentier命名法,将二尖瓣前叶分为A1、A2及A3亚区,后叶分为P1、P2及P3亚区。经食管超声心动图技术采集中食道四腔心切面、中食道瓣膜联合面、中食道两腔心切面、中食道左心室长轴等切面进行瓣叶亚区定位;外科医生术中记录二尖瓣脱垂A1、A2、A3亚区及P1、P2、P3亚区等病变具体部位;同时初步研究二尖瓣脱垂不同亚区与二尖瓣反流之间的相互关系。结果与外科手术相比较,经胸超声心动图技术及经食管超声心动图技术定性诊断二尖瓣脱垂准确率100%,而定位诊断二尖瓣脱垂亚区准确率分别为80%及94%,经胸超声心动图技术及经食管超声心动图技术两种方法之间的检查结果有统计学差异(P〈0.05);二尖瓣反流程度与二尖瓣脱垂不同亚区有关。结论根据二尖瓣叶Carpentier命名法,应用经胸超声心动图技术及经食管超声心动图技术对二尖瓣脱垂部位进行亚区定位,对外科二尖瓣脱垂瓣膜成形术具有重要指导意义,经食管超声心动图技术对二尖瓣脱垂亚区的准确定位较经胸超声心动图技术更有优势;二尖瓣反流程度与二尖瓣脱垂不同亚区有关。
