摘要
目的探讨经胸超声心动图诊断二尖瓣脱垂的临床表现及诊断效能。方法选择46例疑似二尖瓣脱垂患者为研究对象,在手术前均接受经胸超声心动图检查,并经三维图像旋转、切割,确认具体二尖瓣膜脱垂区域,从而判断是否存在腱索断裂,并与手术结果作对照分析。结果经过手术,前叶分区定位中,脱垂患者共有13例,包括A1区4例,A2区5例,A3区4例;后叶分区定位中,脱垂患者共有13例,包括P1区4例,P2区4例,P3区5例。经胸超声心动图检出前叶分区定位中脱垂的有21例;后叶分区定位中脱垂的有16例。经胸超声心动图诊断二尖瓣各区域脱垂,A3和P3区的灵敏度高于A1、A2、P1、P2区;A1和P2区的特异度和准确度高于A2、A3、P1、P3区。经胸超声心动图诊断腱索断裂的灵敏度为100.00%,特异度为80.77%。结论经胸超声心动图可以高效地、快速地、准确地进行二尖瓣脱垂诊断。
Objective To explore the clinical manifestations and diagnostic efficacy of transthoracic echocardiography in the diagnosis of mitral valve prolapse.Methods Forty-six patients with suspected mitral valve prolapse were selected as the study objects.Before the operation,all of patients were examined by transthoracic echocardiography,and the specific mitral membrane prolapse areas were identified by three-dimensional image rotation and cutting,so as to determine whether there was rupture of chordae tendineae,and the results were compared with the results of surgery.Results After surgery,there were 13 patients with prolapse in the anterior lobe area localization,including 4 patients in A1 area,5 patients in A2 area and 4 patients in A3 area;there were 13 patients with prolapse in the posterior lobe area localization,including 4 patients in P1 area,4 patients in P2 area and 5 patients in P3 area.There were 21 patients of prolapse in the anterior lobe area localization detected by transthoracic echocardiography,there were 16 patients of prolapse in posterior lobe localization.In the diagnosis of mitral valve prolapse by transthoracic echocardiography,the sensitivity of A3 and P3 areas were higher than that of A1,A2,P1 and P2 areas;the specificity and accuracy of A1 and P2 areas were higher than those of A2,A3,P1 and P3 areas.The sensitivity and specificity of rupture of chordae tendineae in transthoracic echocardiography were 100.00%and 80.77%respectively.Conclusion Transthoracic echocardiography can evaluate mitral valve prolapse efficiently,quickly and accurately.
