脂蛋白相关磷脂酶A2活性水平的性别差异及其与心血管病危险因素的关系 被引量:31

Impact of gender on lipoprotein-associated phospholipase A2 activity and association with known cardiovascular risk factors

摘要 目的 探讨脂蛋白相关磷脂酶A2(Lp-PLA2)活性水平的性别差异及其与心血管病危险因素的关系.方法 以参加“中国多省市心血管病危险因素队列研究”2002年危险因素调查的1541例北京地区45 ~ 74岁队列人群中资料完整的1471人(男性688人,女性783人)作为研究对象.Lp-PLA2活性水平采用比色法进行检测.对男女两性的Lp-PLA2活性水平以及Lp-PLA2活性与危险因素的关系进行了分析.结果 (1)男女两性Lp-PLA2活性水平分别为(22.73±8.52) nmol·min-1 ·ml-1和(20.01±8.06) nmol·min-1·ml-(P<0.01).(2)男性的年龄、腰围、收缩压、舒张压以及高血压患病率高于女性,女性的总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)高于男性(P<0.05或P<0.01).(3)Pearson相关分析显示,男性Lp-PLA2活性水平与肥胖指标(腰围和体质指数)、血压(收缩压和舒张压)、血脂(TC、LDL-C、HDL-C和甘油三酯)均相关(P均<0.01);女性Lp-PLA2活性水平与年龄和血脂相关(P<0.05或P<0.01),而与肥胖指标和血压的相关性低于男性(女性r值0.02~0.08;男性r值0.10~0.16).多因素logistic回归分析调整年龄、腰围、收缩压、空腹血糖、LDL-C、HDL-C、甘油三酯和高敏C反应蛋白后,男性Lp-PLA2活性水平仍高于女性(OR=1.72,95%CI:1.34 ~2.21,P<0.01).结论 男性的Lp-PLA2活性水平高于女性,且与主要心血管病危险因素的关系较强,但此性别差异并不能够完全由主要危险因素解释. Objective To explore the impact of gender on lipoprotein-associated phospholipase A2 (Lp-PLA2 ) activity and association with known cardiovascular risk factors. Methods Participants in this study were recruited from Beijing sub-cohort from the Chinese Multi-provincial Cohort Study (CMCS) database. A total of 1471 participants with eomplete laboratory data were included in the study (688 male). Lp-PLA2 activity was determined by colorimetric assay kit. Lp-PLA2 activity level and correlation between Lp-PLA2 activity and known risk factors were compared between men and women. Results ( 1 ) Lp-PLA2 activity was higher in males than in females [ (22. 73± 8. 52) nmol · min-1· m1-1 vs. (20. 01± 8.06) nmol ~ rain-l · ml-1, P 〈 0.01 ]. (2) Age, waist circumference, systolic blood pressure, diastolic blood pressure and the prevalence of hypertension were higher in males than in females, while total cholesterol, low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) were higher in females than in males( P 〈 0. 05 or P 〈 0. 01 ). (3)Pearson correlation showed that Lp-PLA2 activity was correlated with lipids ( total cholesterol, LDL-C, HDL-C, and triglyceride), blood pressure ( systolic blood pressure and diastolic blood pressure), and adiposity associated parameters (waist circumference and body mass index) in males( all P 〈 0. 01 ) and was correlated with lipid level (total cholesterol, LDL-C, HDL-C, and triglyeeride) and age in females ( P 〈 0. 05 or P 〈 0. 01 ). Correlations with variables associated with obesity or blood pressure in females were much weaker than those in males ( in females, r = 0. 02 - 0.08 ; in males, r = 0. 10 - 0. 16 ) . (4) After adjustment for age, waist circumference, systolic blood pressure,glucose, LDL-C, HDL-C, triglyceride and high sensitivity C-reactive protein by multiple logistic regression model, Lp-PLA2 activity was still significantly higher in males than in females ( OR = 1.72, 95% confidence interval = 1.34 - 2. 21, P 〈 0. 01 ) . Conclusions Lp-PLA2 activity and association with known cardiovascular risk factors differed in males and females. The gender difference in Lp-PLA2 activity still presents after adjustment for known cardiovascular risk factors in this cohort.
