摘要
目的 观察非肝硬化性胆道梗阻 (BDO)对大鼠 70 %肝部分切除 (PH)术后肝细胞再生的影响。方法 Wistar大鼠分为正常肝部分切除组 (N PH)、胆道梗阻胆流再通 (RBF)肝部分切除组 (BDO RBF PH)及胆道梗阻胆流再通组 (BDO RBF)。BDO RBF PH组大鼠胆总管梗阻 1周后行胆流再通及 70 %PH。免疫组化法检测再生肝细胞增殖细胞核抗原 (PCNA)及溴脱氧核苷尿嘧啶 (BrdU )标记 ,Western Blot法检测肝细胞PCNA蛋白表达 ,RT PCR与ELISA法检测肝内TNFαmRNA与TNFα表达。结果 N PH组肝细胞PCNA表达及BrdU阳性标记指数于 70 %PH术后 2 4h达到高峰 ,而BDO RBF PH组肝细胞DNA合成高峰延后至术后 48h ,其PCNA高峰表达量及BrdU高峰标记指数均低于N PH组 ;BDO RBF PH组大鼠 70 %PH后 0~ 72h肝内TNFαmRNA及TNFα表达量显著高于N PH组。结论 非肝硬化性胆道梗阻胆汁淤积大鼠 70 %PH术后肝细胞再生受抑制 。
Objective To investigate the effect of noncirrhotic biliary duct obstruction (BDO) on the liver regeneration after 70% partial hepatectomy (PH) in rats. Methods The rats were divided into normal PH group (N PH), BDO RBF group in which restoration of bile flow (RBF) was obtained after 1 week's common bile duct obstruction, and BDO RBF PH group in which 70% PH was performed after the same procedures as BDO RBF group. The expression of proliferating cell nuclear antigen (PCNA) and labeling of bromodeoxyuridine (BrdU) were determined with immunohistochemical methods in the regenerating hepatocytes. The expressions of PCNA protein, TNFα mRNA and TNFα were measured with Western Blot, RT PCR and ELISA respectively. Results The expression of PCNA and labeling of BrdU reached the peak at 24 h after PH in N PH group, and at 48 h in BDO RBF PH group, and the peak levels of PCNA and BrdU of hepatocytes in BDO RBF PH rats were decreased significantly compared with those of N PH group. The expression levels of TNFα mRNA and TNFα were significantly higher in BDO RBF PH rats than in N PH group from 0~72 h after 70% PH. Conclusion The hepatocyte proliferation is inhibited in the rats with BDO and 70% PH, and the increased expression of TNFα may be one of the causes of the inhibited liver regeneration.
