摘要
目的 应用保尔佳 (Polyerga)体外净化自体骨髓移植治疗急性白血病及实体瘤 ,探索有效的体外净化新方法。方法 取正常骨髓加入不同浓度的保尔佳在 3 7℃条件下孵育后台盼蓝染色 ,计数活细胞数 ,细胞培养计数CFU -GM ,评价保尔佳对正常造血干细胞的影响。临床应用按 10 0～ 15 0ng/ml保尔佳体外骨髓净化处理 ,净化组预处理方案及回输自体骨髓细胞量与对照组相似。结果 不同浓度的保尔佳在体外对正常造血干细胞均无影响 ,净化组与对照组自体骨髓移植造血恢复时间相似 ,实验组与对照组间无差异 (P >0 0 5 )。随访报告提示 ,净化组复发率较对照组明显降低 (P <0 0 1)。结论 保尔佳体外净化自体骨髓对造血干细胞无损伤 ,可有效清除体内残留肿瘤细胞 ,延长无病生存期。
Objective Purging auto bone marrow in vitro with Polyerga treating acute leukemia and solid tumor,aims at the research of effective new method of purging auto transplantation.Methods Polyerga was mixed in bone marrow sample in different concentration and incubated in the temperature of 37 ℃,then carry out trypan-blue staining and CFU-GM culture to evaluate the effect of Polyerga to hematopoietic stem cell.In clinical application,Polyerga in a dosage of 100～150 ng/ml was mixed in harvest auto bone marrow.Conditioning regiment and the number of cells for engraftment were just the same in therapeutic and control groups.Results Polyerga in different concentration had no bad effect in vitro in hemotopoietic stem cell,recovery periods of hematopoiesis between the two groups were similar.The follow-up observation suggested that recurrent rate of malignant tumor in the purging group was obviously lower than that in control group ( P <0 01).Conclusions Auto transplantation purging with Polyerga can successfully eliminated MRD,to give prolongation of DFS
