膜性肾病患者尿蛋白成分与临床及病理的相关性分析 被引量:3

The relationship between urinary protein components and clinical and pathological characteristics in membranous nephropathy patients

摘要 目的 探讨膜性肾病(MN)患者尿中N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、视黄醇结合蛋白(RBP)、白蛋白(ALB)、免疫球蛋白G(IgG)、补体C3的构成特点及其与临床和病理的关系.方法 收集原发性膜性肾病患者95例,测定患者尿液中NAG、RBP、ALB、IgG及C3的浓度,并与各项临床及病理指标进行统计分析.结果 51 ~ 70岁年龄组患者尿RBP、IgG及C3含量较14~ 20岁组明显升高(P<0.05).血压升高组较血压正常组各种尿蛋白成分浓度均明显升高(P<0.05).肾功能不全组患者的尿NAG、RBP、ALB及IgG及补体C3水平均高于肾功能正常组(P<0.05).伴镜下血尿组患者尿RBP、ALB及IgG较不伴镜下血尿组明显增高(P<0.05).随着病理分期进展,患者尿NAG、RBP、IgG和C3等浓度明显升高(P<0.05);随着小管间质损害的加重,尿NAG、IgG和C3浓度均明显升高(P<0.05).结论 膜性肾病患者尿NAG、RBP、ALB、IgG及C3检测可以反映患者的病情,提示肾脏病理损害程度. Objective Explore the significance of urine N-acetyl-[3-D-glucosaminidase(NAG), retinol-binding protein(RBP),albumin(ALB),immunoglobulin G(IgG) and complement component 3 ( C3 ) , and investigate their relationship with clinical-pathological characteristics in membranous nephropa- thy patients. Methods Ninety five patients diagnosed with membranous nephropathy by renal biopsy were recruited. Urinary samples were collected to analyze the protein components. Results The levels of urina- ry RBP ,IgG and C3 in group aged 51-70 years old were significantly higher than those in group aged 14-20 years old. All the urinary components were obviously increased in the group with hypertension. Group with microscopic hematuria had much higher levels of urinary RBP,ALB and IgG than patients without micro- scopic hematuria. The levels of urinary NAG, RBP, ALB and C3 were obviously increased in the group with renal dysfunction. With the progress of pathological stage, the levels of urinary NAG, RBP, IgG and C3 were significantly increased. The concentrations of NAG, IgG and C3 increased in group with more tubu- lointerstitial damage. Conclusion The measurement of urinary NAG, RBP, ALB, IgG and C3 may reflect the progress of disease and suggest the extent of damage in renal pathology, and it can determine the dis- ease prognosis and guide treatment.
