摘要
目的探讨子宫内膜异位症(EM)患者血清细胞因子与CA125水平的相关性。方法选择自2010年1至2013年1月定州市妇幼保健院收治的经腹腔镜确诊的子宫内膜异位症患者81例为观察组,同时选择健康女性40例为对照组。对两组研究对象的白介素(IL)6、IL-8、血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)及肿瘤标志物CA125、性激素水平进行测定,并进行比较。结果按美国生育学会分期法分期的Ⅰ期患者IL-6、IL-8、VEGF、TNF-α较对照组差异无统计学意义(均P>0,05);Ⅱ期患者IL-6、IL-8、TNF-α较对照组均出现明显升高(t值分别为7.317、5.391、6.724,均P<0.05);Ⅲ期患者IL-6、IL-8、VEGF、TNF-α较对照组有显著增高(t值分别为5.826、6.761、7.827、8.293,均P<0.05),较Ⅰ期患者差异亦有统计学意义(t值分别为6.391、6.322、5.285、4.983,均P<0.05);V期患者IL-6、IL-8、VEGF、TNF-α较对照组及Ⅰ期患者具有显著性增高(t值分别为5.837、7.291、6.832、6.925;6.523、7.264、6.842、5.883,均P<0.05),IL-6、IL-8、VEGF较Ⅱ期患者出现显著性增高(t值分别为5.187、4.862、5.524,均P<0.05)。Ⅰ、Ⅱ期患者CA125较对照组均出现显著性升高(t值分别为6.829、5.416,均P<0.05),Ⅲ期患者CA125较对照组有显著增高(t=8.761,P<0.01),Ⅳ期患者CA125较对照组及Ⅰ期患者显著性增高(t值分别为7.253、5.817,均P<0.05)。CA125与IL-6、IL-8及VEGF呈显著正相关(r值分别为0.507、0.467、0.526,均P<0.05),TNF-α与CA125无显著相关性(P>0.05)。结论 CA125与细胞因子水平增高对判断子宫内膜异位症分期的进展具有重要意义,且具有较好的相关性。
Objective To investigate the correlation between serum cytokines and CA 125 level in patients with endometriosis ( EM ) . Methods From January 2010 to January 2013 81 EM patients, diagnosed by laparoscopy in Dingzhou Maternal and Child Health Hospital , were selected in observation group , and another 40 cases of healthy women at the same period were included in control group .IL-6, IL-8, VEGF, TNF-α, CA125 and sex hormone levels of two groups were measured and compared .Results In the observation group patients at stage I, staged according to American Fertility Society staging method , were not significantly different from control group in IL-6, IL-8, VEGF and TNF-α(all P〉0.05).Patients at stage Ⅱhad obviously higher levels of IL-6, IL-8 and TNF-αthan control group (t value was 7.317, 5.391 and 6.724, respectively, all P〈0.05).Compared with the control group, patients at stage Ⅲhad significantly higher levels of IL-6, IL-8, VEGF and TNF-α( t value was 5.826, 6.761, 7.827 and 8.293, respectively, all P〈0.05), and there was statistical significance between patients at stageⅠand at stage Ⅲ(t value was 6.391, 6.322, 5.285 and 4.983, respectively, all P〈0.05).The levels of IL-6, IL-8, VEGF and TNF-αin patients at stage Ⅳ were significantly higher than those in control group and the patients at stage Ⅰ(t value was 5.837, 7.291, 6.832 and 6.925;6.523, 7.264, 6.842 and 5.883, respectively, all P〈0.05), and the levels of IL-6, IL-8 and VEGF were obviously higher than those in patients at stage Ⅱ ( t value was 5.187, 4.862 and 5.524, respectively, all P〈0.05).The CA125 level in patients at stage ⅠandⅡwas significantly higher than control group (t value was 6.829 and 5.416, respectively, both P〈0.05), and that in patients at stage Ⅲ was obviously higher than the control group (t=8.761,P〈0.01).Compared with control group and patients at stage Ⅰ, the CA125 level in patients at stage Ⅲincreased significantly (t value was 7.253 and 5.817, respectively, both P〈0.05).CA125 was positively related with IL-6, IL-8 and VEGF (r value was 0.507, 0.467 and 0.526, respectively, all P〈0.05), but it was not closely related with TNF-α(P〉0.05).Conclusion The increasing of CA125 level and cytokine level is of great significance in judging the progress of EM stage , and there is a good correlation between them .
