糖尿病大鼠心肌缺血预处理作用减弱与NO-cGMP信号通路抑制的关系

摘要 目的探讨缺血预处理(IPC)对糖尿病大鼠心肌有无保护作用及其可能机制。方法糖尿病及非糖尿病SD大鼠各30只,建立在体阻断左冠状动脉模型。各分为3组(n=10):假手术组(Sham组)、缺血再灌注组(I/R组)及IPC组。比较各组大鼠血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)及乳酸脱氢酶(LDH)含量,以及心肌组织丙二醛(MDA)、超氧化物歧化酶(SOD)、环磷酸鸟苷(cGMP)、一氧化氮(NO)、一氧化氮合酶(NOS)含量的变化。制作心肌组织电镜切片,对心肌线粒体行Flameng评分。结果非糖尿病IPC组与I/R组比较心肌酶漏出明显减少,MDA含量降低,线粒体损伤减轻,SOD、cGMP、NO、NOS含量增加(P<0.05),而IPC在糖尿病大鼠未表现出明显心肌保护作用,糖尿病IPC组心肌组织cGMP、NO、NOS含量均无明显增加(P>0.05)。结论 IPC的作用在糖尿病大鼠心肌中明显减弱,其机制可能与NO-cGMP信号通路在糖尿病心肌中的表达受到抑制有关。 Objective To explore the effect of ischemic preconditioning (IPC) on myocardial preservation in diabetes mellitus (DM) rats and the possible mechanism. Methods After the reproduction of left coronary artery block model, 30 DM and 30 non-DM SD rats were respectively divided into 3 groups (n=10), including the sham group, ischemia/reperfusion (I/R) group and IPC group. The contents of serum creatine kinase (CK), creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), myocardial malondialdehyde (MDA), superoxide dismutase (SOD), cyclic guanosine monophosphate (cGMP), nitrogen monoxide (NO) and nitric oxide synthase (NOS) were determined. Flameng score was used to evaluate the structure of the mitochondria. Results Myocardial enzyme leakage and mitochondrial injury were reduced significantly in IPC group compared with I/R group in non-DM rats, while SOD content, cGMP, NO and NOS levels were increased significantly (P〈0.05). IPC showed no significant effect on myocardial preservation, and cGMP, NO and NOS did not show a significant increase in DM rats (P〉0.05). Conclusion The effect oflPC is reduced in hearts ofDM rats, which may be related to the suppression of NO-cGMP signal pathway.
