摘要
目的观察圆锥角膜的超微结构变化,拟发现圆锥角膜的特异性改变,为探讨其发病机制提供依据。方法行穿透角膜移植术的圆锥角膜片20例,分别用光镜和电镜观察其各层次的改变。结果圆锥角膜上皮层、基质层变薄,基底膜及前弹力层局限性断裂或破坏;上皮基底细胞水肿,角膜细胞形态、结构改变,并似有异常吞噬现象;基质排列紊乱,板层间可见微粒状沉积物。结论提示圆锥角膜的原发病变可能在角膜基质细胞。
ObjectiveTo explore specific changes and to provide evidence for pathogenesis of keratoconus by examining its ultrastructure changes.MethodsTwenty corneal buttons of keratoconus were obtained from penetrating k eratoplasty and changes of every layer w ere observed by light and electron microscopy.Re sultsThe findings were as follows:thin e pithelium and stroma,local rupture and d estruction in basement membranes and Bowm an's layer,basal cell swollen,deformity and structural changes of keratocytes wit h abnormal phagocytosis,stroma disarrange ment,granular deposition among laminae. Conclusion Primary disorder of the keratoconus may lie keratocytes.
