摘要
目的:探讨中医学理论中的"血痹"学说理论与奥沙利铂引起的周围神经毒性的相关性及黄芪桂枝五物汤对该神经毒性的治疗作用的有效性及安全性。方法:计算机检索中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、万方数据在线知识服务平台、中文科技期刊全文数据库(VIP)以及Cochrane Library、Medcine、EMbase英文数据库,按照纳入和排除标准筛选文献和提取资料后,采用RevMan 5.0软件进行统计分析。结果:①共纳入6个随机对照实验研究,368例患者。Meta分析结果显示化疗时使用黄芪桂枝五物汤的试验组对比常规治疗组或甲钴胺片治疗组在神经毒性的发生率、严重神经毒性的发生率及感觉神经传导速度(SNCV)等方面具有统计学差异。②安全性分析纳入黄芪桂枝五物汤治疗奥沙利铂引起的外周神经毒性的所有类型的临床研究,结果未发现严重的不良事件(ADR/AE)。结论:辨证应用黄芪桂枝五物汤可以预防奥沙利铂引起的周围神经毒性,降低严重周围神经毒性的发生率,并可以缓解奥沙利铂导致的感觉神经传导速度的抑制作用,且根据临床实验研究报道,运用黄芪桂枝五物汤无明显不良反应事件的发生,具有一定的使用安全性。
Objective：To investigate the links between the ‘ blood arthralgia＇ theory of Chinese medicine and Oxaliplatin-induced peripheral neurotoxicity,and the effectiveness and severity of Huangqi Guizhi Wuwu decoction on the effect for the treatment of Oxaliplatin-induced peripheral neurotoxicity.Method：Completely searching the clinical researches of Huangqi Guizhi Wuwu decoction for Oxaliplatin-induced peripheral neurotoxicity by computer from CBM,CNKI,Wanfang Data,VIP,Cochrane Library,Medcine,EMbase by the inclusive and exclusive criteria.RevMan 5.0 was used for Metaanalysis.Result：①Based on the outcomes of 368 patients,16 clinical trials were identified to meet the inclusion criteria.The meta-analyses showed statistically significant differences in favor of Huangqi Guizhi Wuwu decoction compared with mecobalamin for the effectiveness on the effect for the treatment of the peripheral neurotoxicity induced by Oxaliplatin,including the incidence and severity of peripheral neurotoxicity and SNCV,compared with the conventional therapy or mecobalamin.② For all researches the safety analysis found no serious adverse events or adverse reactions （ADR/AE）.Conclusion：Huangqi Guizhi Wuwu decoction can prevent Oxaliplatin-induced peripheral neurotoxicity,reduce the incidence of severe peripheral neurotoxicity and alleviate the inhibition of SNCV of oxaliplatin.According to the clinical experimental research,there is no serious adverse events or adverse reactions （ADR/AE） of Huangqi Guizhi Wuwu decoction,so Huangqi Guizhi Wuwu decoction has a certain degree of security for clinical application.
