摘要
目的:检测慢性乙型肝炎孕妇新生儿脐带血T淋巴细胞亚群、HBV标志物和孕妇肝功能的变化,探讨孕妇乙型肝炎病毒(HBV)感染对新生儿脐血T淋巴细胞亚群的影响。方法:选择38例HBV携带者产妇为观察组,15例HBsAg阴性健康产妇作为对照组,收集产妇静脉血和脐带血。检测脐血T淋巴细胞亚群(包括CD3+,CD4+,CD8+,CD19+,CD16+CD56+,CD4+/CD8+)的百分比、孕妇HBV标志物、脐血的HBV标志物。分析比较脐血不同HBV标志物模型与T淋巴细胞亚群的关系及T淋巴细胞亚群与肝功能的相关性。结果:①感染组和对照组脐血HBV标志物各模式对其脐血T淋巴细胞亚群无明显影响,差异无统计学意义(P>0.05)。②感染组和对照组肝功能对脐血的细胞免疫无明显影响,差异无统计学意义(P>0.05)。③HBsAg阴性健康产妇组与HBeAg阳性HBsAg携带者、HBeAg阴性HBsAg携带者分娩的新生儿细胞免疫状况比较,差异均无统计学意义(P>0.05)。结论:①HBV孕妇所分娩的新生儿脐血T淋巴细胞亚群无比例失衡。②脐血T淋巴细胞之间有内在的协助与互相制约的动态平衡关系。
Objective: To detect T lymphocyte subgroup and hepatitis B virus (HBV) markers in umbilical cord blood and change of liver function of pregnant women with chronic hepatitis B, explore the effect of maternal HBV infection on T lymphocyte subgroup in um- bilical cord blood of neonates. Methods: Thirty - eight pregnant women carrying HBV were selected as observation group and 15 healthy pregnant women canoeing negative HBsAg were selected as control group; the venous blood specimens of pregnant women and umbilical cord blood specimens of neonates were collected; the proportions ofT lymphocyte subgroups (including CD3+, CD4+, CD8+, CD19+, CD16+ CD56+ , CD4 +/CD8 + ), maternal HBV markers, and HBV markers of umbilical cord blood were detected; the relationship between differ- ent HBV marker models in umbilical cord blood and T lymphocyte and the correlation between T lymphocyte subgroup and liver function were analyzed and compared. Results: In infection group and control group, various HBV marker models in umbilical cord blood had no signifi- cant impact on T lymphocyte subgroup in umbilical cord blood, there was no statistically significant difference (P 〉 0. 05) ; live function had no impact on cellular immunity in umbilical cord blood, there was no statistically significant difference ( P 〉 0.05 ) ; there was no statistically significant difference in cellular immunity among the healthy pregnant women with negative HBsAg, the pregnant women with positive HBeAg and calxying HBsAg, the pregnant women with negative HBeAg and carrying HBsAg (P 〉 0. 05 ) . Conclusion: No imbalance of proportion of T lymphocyte subgroup is found in umbilical cord blood of neonates born by pregnant women with HBV ; there is an internal assistant and coinpeting dynamic balance among the T lymphocytes in umbilical cord blood.
