摘要
目的:观察复方百蛇洗剂对解脲支原体宫颈炎患者阴道局部TNF-α和IL-8的影响,探讨其对解脲支原体宫颈炎患者阴道局部免疫状态的改善。方法:将解脲支原体宫颈炎患者180例,随机分为中药组、西药组和中西医结合组各60例,中药组予复方百蛇洗剂阴道灌洗每天1次,7 d为1疗程;西药组予常规强力霉素口服,7 d为1疗程;中西医结合组予复方百蛇洗剂阴道灌洗常规联合强力霉素口服,7 d为1疗程。应用酶联免疫吸附法检测治疗前后阴道灌洗液中IL-8和TNF-α的水平。结果:3组治疗后阴道灌洗液中TNF-α水平均显著低于治疗前,差异有统计学意义(P<0.05);治疗后中西医结合组阴道灌洗液中TNF-α水平明显低于中药组和西药组,差异有统计学意义(P<0.05);3组治疗后阴道灌洗液中IL-8水平均低于治疗前,但差异均无统计学意义(P>0.05);治疗后3组间阴道灌洗液中IL-8水平差异无统计学意义(P>0.05);中西医结合组解脲支原体转阴率(58.33%)明显高于中药组(23.33%)和西药组(38.33%),差异有统计学意义(P<0.05)。结论:复方百蛇洗剂与强力霉素联合治疗解脲支原体宫颈炎可能通过改善阴道局部的免疫状态起协同作用而提高疗效,阴道局部TNF-α的水平可能成为药物疗效的判定指标之一。
Objective ： To observe the effects of Compound Baishe Lotion on the expressions of TNF - αand IL - 8 of vaginal in ureaplasma urealyticum cervicitis patients, and to explore the improvement of vaginal immune status. Methods： Totally 180 cases of ureaplasma urealyticum cervicitis were randomized into three groups： Chinese medicine group in which 60 cases were treated with Compound Baishe Lotion, western medicine group in which 60 cases were treated with o- ral doxycycline, and combination group in which 60 cases were treated with Compound Baishe Lotion and doxycycline oral- ly. IL - 8 and TNF -α levels of vaginal fluid were observed with enzyme - linked immunosorbent assay before and after treatment. Results ： After treatment, TNF -α levels in vaginal lavage fluid of three groups were significantly lower than be- fore （ P 〈 0.05 ）. TNF - α level in vaginal lavage fluid of combination group after treatment was significantly lower than that of Chinese group and western medicine group after treatment （ P 〈 0.05 ）. After treatment IL - 8 levels in vaginal lavage fluid of three groups were lower than before, but the difference was not statistically significant （ P 〉 0.05 ） ; after treatment IL - 8 level in vaginal lavage fluid had no significant difference among three groups （P 〉 0.05 ） ; UU negative rate in combination group （58.33%） was significantly higher than that of medicine group （23.33%） and western medi- cine group （ 38.33 % ）, and the difference was statistically significant （ P 〈0. 05 ）. Conclusion ： The therapy of Compound Baishe Lotion and oral doxycycline may improve efficacy by improving the immune status, and TNF -α level may be one of the indicators to determine efficacy.
