摘要
目的探讨血清同型半胱氨酸(homocysteine,Hcy)、脂蛋白(a)[lipoprotein(a),Lp(a)]水平的变化与老年高血压病患者颈动脉粥样硬化程度的关系。方法将60例高血压病患者根据颈动脉粥样硬化的程度分为3组:颈动脉内膜中层厚度(intima-media thickness,IMT)正常组(IMT≤0.9mm,颈动脉IMT正常组,16例)、颈动脉IMT(IMT 1.0~1.2mm)增厚组(19例)和颈动脉IMT(IMT≥1.3mm)斑块形成组(25例)。使用全自动生化分析仪、采用乳胶浊度法测定3组患者Lp(a)水平;使用全自动免疫分析仪、采用化学发光法测定3组患者Hcy水平。结果颈动脉IMT增厚组、颈动脉IMT斑块形成组血清Hcy、Lp(a)水平均明显高于颈动脉IMT正常组(均P〈0.05),颈动脉IMT斑块形成组血清Hcy、Lp(a)水平均明显高于颈动脉IMT增厚组(均P〈0.05)。颈动脉IMT增厚组、颈动脉IMT斑块形成组血清Hcy水平与血清Lp(a)水平均呈正相关(r=0.65、0.64,均P〈0.05)。结论老年高血压病患者血清Hcy、Lp(a)水平升高与颈动脉粥样硬化密切相关。
Objective To investigate the relationship between intima-media thickness(IMT) and levels of serum homocysteine(Hcy)and lipoprotein(a)[Lp(a)]in elderly patients with hypertension.Methods Sixty elderly hypertensive patients were divided into three groups according to the degree of carotid artery atherosclerosis:normal group(IMT≤0.9mm,n=16),thickening group(IMT1.0-1.2mm,n=19)and plaque formation group(IMT≥1.3mm,n=25).Levels of Lp(a)and Hcy were determined by emulsion turbidity method and chemiluminescence method using an automatic biochemical analyzer,respectively.Results Compared with normal group,Lp(a)and Hcy levels significantly increased in thickening group and plaque formation group(P< 0.05).Compared with thickening group,Lp(a)and Hcy levels significantly increased in plaque formation group(P<0.05).There was a positive relationship between Hcy and Lp(a)in thickening group and plaque formation group(r=0.65and 0.64,respectively;P<0.05).Conclusion The degree of carotid artery atherosclerosis is closely correlated with serum levels of Hcy and Lp(a)in elderly patients with hypertension.
