持续正压通气对中重度阻塞性睡眠呼吸暂停低通气综合征伴糖耐量减退患者血清胰淀素水平的影响 被引量:6

摘要 目的观察持续正压通气(CPAP)对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)伴糖耐量减退患者血清胰淀素水平的影响。方法选择2006—2008年我院诊治的中重度OSAHS合并糖耐量减退患者40例,根据患者是否接受CPAP治疗,将其分为CPAP治疗组和非CPAP治疗组,两组患者均进行常规治疗,在此基础上CPAP治疗组患者采用CPAP治疗。比较两组治疗前、治疗6个月后体质指数(BMI)、腰臀比、IR指标(HOMA-IR)、睡眠呼吸紊乱指数(AHI)、氧减指数(ODI)、氧饱和度低于90%时间百分比(SLT 90%)及空腹血清胰淀素水平。结果治疗6个月后CPAP治疗组HOMA-IR、AHI、ODI、SLT 90%及空腹血清胰淀素水平均低于非CPAP治疗组,差异均有统计学意义(P<0.05)。结论 CPAP治疗OSAHS伴糖耐量减退,随着呼吸暂停次数减少及缺氧时间缩短,可以改善血清胰淀素水平及胰岛素抵抗。 Objective To investigate the effect of continuous positive airway pressure (CPAP) on the level of amylin in obstructive sleep apnea hypopnea syndrome (OSAHS) patients with abnormal glucose tolerance. Methods 40 OSAHS pa- tients with abnormal glucose tolerance were chosen from 2006 to 2008, they were divided into CPAP treatment group and non - CPAP treatment group, according to the patients's acceptance on using CPAP treatment. All patients were given routine treat- ment, on this basis, the CPAP treatment group was given CPAP treatment. The body mass index (BMI) , waist - to - hip rati- o, IR index (HOMA -IR) , sleep respiratory disturbance index (AHI), oxygen reduction index (ODI), the time of oxygen saturation percentage lower than 90% ( SLT 90% ) and serum amylin level of two groups were compared before and after six months treatment. Results After CPAP treatment for 6 months, HOMA -IR, AHI, ODI, SLT90% and serum amylin in CPAP treatment were lower than those of non - CPAP treatment group. Conclusion CPAP in the treatment of OSAHS with im- paired glucose tolerance, which demonstrate that the CPAP treatment can improve insulin resistance by reducing the times of ap- nea hypopnea and shorter of hypoxia.
