摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿血清瘦素(LP)和肿瘤坏死因子-α(TNF-α)水平的变化及其与OSAHS的关系。方法采用放射免疫法检测68例OSAHS患儿及30例正常儿童血清LP和TNF—α水平,比较其组间差异,并分析OSAHS患儿血清LP和TNF—α水平与睡眠呼吸紊乱指数(AHI)和最低血氧饱和度(SαO1)的相关性;OSAHS组患儿进行持续气道正压通气(CPAP)治疗6个月后复查上述各项指标,比较治疗前后血清LP和TNF—α水平的变化。结果OSAHS组血清LP和TNF—α水平较对照组升高(P均〈0.05),OSAHS患儿血清LP和TNF-α水平与AHI呈正相关(r=0.451,0.460,P〈0.05),与其最低SαO2呈负相关(r=-0.489,-0.478,P〈0.05);OSAHS患儿经CPAP治疗后血清LP和TNF—α水平较治疗前减低(P均〈0.05)。结论OSAHS患儿血清LP和TNF-α水平升高与其低氧血症有关;CPAP可改善OSAHS患儿缺氧状态,降低血清LP和TNF—α水平。
Objeαive To explore the change of the serum leptin and tumor necrosis faαor-α levels in children with obstruαive sleep apnea-hypopnea syndrome and their relationship. Methods The level of serum leptin and TNF-α were determined in 68 children with ob- struαive sleep apnea-hypopnea syndrome and 30 healthy children by radio-immunity assay, and the results from patients treated with continuous positive airway pressure (CPAP) for six months were compared. The correlation between leptin and the levels of tumor necrosis faαor-α, and apnea hypopnea index ( AHI ) or the minimum saturation of blood oxygen were analyzed. Results The level of leptin and tumor necrosis faαor-α in OSAHS patients were higher than those in control ( P 〈 0. 05 ). The levels of leptin and tumor necrosis faαor - α were positive correlated with the AHI ( r = 0. 451, r = 0. 460, P 〈 0. 05 ) and negative correlated with the minimum saturation of blood oxygen ( r = - 0. 489, r = - 0. 478, P 〈 0. 05 ). The levels of leptin and tumor necrosis faαor - α were decreased after treatment. Conclusion The levels of leptin and tumor necrosis faαor-α are increased in OSAHS children. The increase of leptin and tumor necrosis faαor-α is correlated with hypoxemia, and the levels of leptin and tumor necrosis factor-α can be improved by CPAP in OSAHS children.
