超声序列扫查法与四腔心筛法筛查胎儿先天性心脏病的效果比较 被引量:1

摘要 目的探讨序列扫查法在诊断胎儿先天性心脏病(先心病)中的价值。方法研究对象330例,分为患者组(70例),正常组(260例),两组分别采用序列扫查法和四腔心筛法进行胎儿先心病的筛查,比较两种方法的灵敏度、特异度、误诊率、漏诊率、准确度、阳性预测值、阴性预测值等。结果统计分析了患者组中胎儿先心病的种类;序列扫查法中三血管切面及三血管气管切面、右室流出道切面在正常组的显示率要显著高于患者组;序列扫查法的灵敏度、准确度、阳性预测值、阴性预测值均高于四腔心筛法,两者的特异度基本一致,漏诊率显著低于四腔心筛法。结论序列扫查法的灵敏度与特异度均高,是筛查胎儿先心病的最佳方法,对产前常规筛查出的胎儿先心病应运用该法进一步检查以明确诊断。 Objective To investigate the value of sequence scanning method in the diagnosis of fetal congenital heart disease. Methods The 330 cases with this disease were divided into case group (70 cases) and normal group (260 cases), all of the patients were respectively subjected to sequence scanning method and the four-chambered heart screening method for screening of fetal congenital heart disease, then the sensitivity, specificity, the rate of misdiagnosis and missed diagnosis rate, accuracy, positive predictive value, negative predictive value of the two methods were compared. Results The kinds of fetal congenital heart diseases were classified in the case group ; the rate of display in three vascular section and three-vessel trachea section, right ventricular outflow tract section of serial scanning method in normal group was significantly higher than the case group; the sensitivity, accuracy, positive predictive value, and negative predictive values in serial scanning method were higher than the four-chambered heart screening method; misdiagnosis rate was significantly lower than the four-chambered heart screening method. Conclusion The sensitivity and specificity of sequence scanning method all are higher compared with another one, it is the best way to analyse fetal congenital heart disease can also be applied to further examine and confirm the diagnosis of suspected cases.
