免疫增强治疗对隐性梅毒患者Th_1/Th_2细胞因子的影响 被引量:8

摘要 目的通过比较隐性梅毒患者免疫增强治疗前后患者血清Th1型细胞因子(IL-2、IFN-γ)和Th2型细胞因子(IL-4、IL-10)水平的变化,探讨常规驱梅联合金菌灵胶囊对隐性梅毒患者血清IL-2、IFN-γ、IL-4、IL-10水平的影响,为临床应用新的治疗方法提供基础理论支持。方法酶联免疫吸附试验(ELISA)分别检测正常组、治疗前后对照组、观察组血清中IL-2、IFN-γ、IL-4、IL-10的水平。对照组仅予苄星青霉素常规治疗;而联合治疗组予苄星青霉素联合金菌灵胶囊治疗。结果隐性梅毒患者血清IL-2、IFN-γ水平显著低于正常对照组(P<0.01);IL-4、IL-10水平显著高于正常对照组(P<0.01)。对照组治疗6个月后血清IL-2、IFN-γ水平均有显著升高(P<0.01),但仍低于对照组(P<0.01);而IL-4、IL-10水平显著降低(P<0.01),但仍高于正常组(P<0.01);但联合治疗组疗3个月和6个月后血清IL-2、IFN-γ水平均显著升高(P<0.01),而血清IL-4、IL-10水平则明显降低(P<0.01),且各因子水平与正常组相比无统计学意义(P>0.05)。结论隐性梅毒患者存在免疫调节功能紊乱,表现为Th2型细胞因子占主导,Th1向Th2漂移状态,这些现象可能是导致TP潜伏感染的主要原因之一。临床上对于隐性梅毒患者,除采用常规的驱梅治疗外,可加用提高细胞免疫功能的药物作为辅助治疗,以调节和增强机体免疫,加快隐性梅毒患者的及早、彻底治愈。 Objective To investigate the effect of conventional treatment combination with JinJunLing Capsule on the serum concentration of IL-2, IFN-γ, IL-4, IL-10 of patients with latent syphilis. Methods Serum samples were obtained from 52 patients with latent syphilis and 20 healthy controls. The patients with latent syphilis were divided into two groups, the control group was treated with benzathine penicillin, and treatment group was treated with benzathine penicillin and JinJunLing Capsule. ELISA method was performed to detect the scum concentrations of IL-2, IFN-γ, IL-4, IL- 10 in normal controls and patients before and after treatment. Results The concentration of IL-2, IFN-γ in patients with latent syphilis were lower than that in normal control significantly (P〈0. 01 ), but the concentration of IL-4, IL-10 in patients with latent syphilis were higher than that in normal control significantly (P〈0. 01 ). After 6 months of treatment with benzathine penicillin, the concentration of IL-2, IFN-γ in treatment group were increased, but were lower than that in normal control group significantly (P〈0. 01 ), while the concentration of IL-4, IL-10 decreased, but were higher than that in normal control group significantly ( P〈0. 01 ). After 3 and 6 months of treatment with benzathine penicillin, the concentration of IL-2, IFN-γ in combined treatment group were increased and the concentration of IL-4, IL-10 were decreased significantly (P〈0. 01 ). Compared with normal controls, there have no significant difference (P〉0. 01 ). Conclusion The patients with latent syphilis may have immune function disorder, it is noted that Th2 plays an important role and Th1 drifts to Th2. This phenomenon may be one of main reasons of TP latent infection. Conventional treatment combination with immunopotentiation therapy is an an effective method in treatment of latent syphilis.
