摘要
:目的 阐明颧颊翼咽鼻突义颌修复双侧上颌骨部分缺损前的外科准备。方法 对 1992年以来的 7例患者实施了修复前外科手术 ,所有病例均存留部分上颌骨颧突及颧骨上颌突 ,进行口鼻道成形术以利于义颌前方固位 ,必要时在腭部行就位道成形术以利于义颌后方固位 ,在颧颊部进行颧颊承力区成形术为义颌创建承力支持部位。结果 各类手术均获得满意效果 ,术后义颌修复使病人的外形和咀嚼及发音等功能均有所改善。结论 颧颊翼咽鼻突义颌修复以部分硬腭存留和软腭保持连续者修复效果较好 。
Objective We studied the operating methods for reconstructing bearing and retaining area of bilateral maxillary defects repaired by the large maxillary palatal prosthodontic appliance with zygomatic buccal wings and pharyngeal nasal processus.Methods Seven patients with bilateral maxillary defects received the operations for prosthetic repair. The remainder of the maxillary zygomatic process and the zygomatic maxillary process could be utilized in all patients. The plastic operation of oral nasal path of insertion of the nasal process was done for using nasal vestibulum as anterior stability of the prosthetic appliance, and in the light of palatal condition the plastic operation of palatal path of insertion of pharyngeal process was performed for using pharyngeal cavity as posterior stability of the prosthetic appliance. The zygomatic buccal bearing area plastic operation was done for establishing proper supporting structures for subsequent placement of the prosthetic appliance.Results The prothetic restoration was completed after healing of the operations. The stability of the prosthetic appliances was good. The restoration of masticatory function and voice, combined with restoration or improvement of dental and facial esthetics was satisfactory following prosthetic rehabilitation. The maxillary palatal prosthodontic appliance with zygomatic buccal wings and pharyngeal nasal processus is effective measurements for repairing large bilateral maxillary defects.Conclusion For the prosthetic reconstruction better prosthetic results can be obtained in the condition of remaining hard palate and continuing soft palate, and it is beneficial to use the remaining maxillary zygomatic process and zygomatic maxillary process to offer bearing area in any type defects.
