摘要
目的观察益生菌肠内营养在危重症脑卒中患者治疗的效果。方法选择120例危重症脑卒中患者,按入院顺序随机分成研究组(EN+益生菌组)61例和对照组(EN组)59例。两组患者均采用肠内营养支持,入院72 h内开始经鼻饲泵入,研究组在此基础上每日肠内营养之前半个小时,经鼻胃管给予益生菌活菌制剂250 mL,2次/d,连用1周。观察两组患者营养支持阶段胃肠道不良反应(腹胀、腹泻、便秘、胃潴留、反流)及达到肠内营养目标供给量所需的时间和住院期间感染并发症发生情况、营养指标等。结果试验组患者胃肠道不良反应率明显低于对照组(P<0.05)。试验组患者达到肠内营养目标供给量所需的平均时间明显快于对照组(P<0.05)。试验组泌尿系统感染例数少于对照组(P<0.05)。两组肺部感染、应激性溃疡等与对照组相比差异没有统计学意义(P>0.05)。结论联合益生菌制剂的肠内营养治疗方法可有效改善危重症脑卒中患者胃肠动力障碍、提高重症脑卒中患者对肠内营养的接受程度,能够预防和减少感染性并发症,从而提高治疗效果。
Objective Toobserve probiotics enteral nutrition in critically ill patients with stroke treatment effect. Methods 120 cases of critically ill patients with stroke, according to order of admission were randomly divided into study group (EN + probiotic) 61 cases and the control group (EN group) of the 59 cases. Two groups of patients were treated with enteral nutritional support, started within 72 hours after admission, nasogastric pumped, study group on the basis of half an hour before the daily enteralnutrition through nasogastric tube probiotic bacteria preparation 250 mL, 2 times/d. Nutritional were observed phase gastrointestinal adverse reactions (bloating, diarrhea, constipation, support gastric retention, reflux) and the incidence of infectious complications and enteral nutrition to achieve target supply amount, the time required. Result Experimental group were gastrointestinal adverse reactions was significantly lower than the control group (P 〈 0.05 ). Experimental group patients achieved enteral nutrition goals average time required to supply significantly faster than the control group ( P 〈 0. 05 ) Experimental group the number of cases of urinary tract infections less than the control group (P 〈 0. 05). Two groups of lung infection, stress ulcer compared with the control group was not statistically significant (P 〉 0. 05 ). Conclusions Probiotics enteral nutrition therapy can effectively improve the critically ill patients with stroke gastrointestinal motility disorder, improve severe stroke patients received enteral nutrition levels, we can prevent and reduceinfectious complications, thereby improving treatment results.
