摘要
目的观察应用羟基磷灰石涂层股骨柄行全髋人工关节置换的疗效。方法选择2007年4月—2010年11月使用Corail全羟基磷灰石涂层股骨柄进行人工全髋关节置换术141例(164髋),观察疗效及随访情况。结果最终共获得随访107例,平均随访47个月,围术期发生术中经股骨矩骨折6例,均对症治疗后痊愈;术后感染2例,1例保守治疗,1例早期清创,关节保留,随访2年以上无复发。影像检查结果无假体松动下沉、无假体周围骨折、无迟发感染、无大腿痛发生、无关节脱位。15髋柄假体周围出现点片状密度增高区。至随访终期Harris评分明显改进(P<0.01)。结论 Corail柄应用具有操作简单、安全性高、长期效果稳定和应用范围广泛的特点,正确全面对其特点的认识与良好的手术技术是获得长期效果的基础。
Objective To observe the curative effect of total hip arthroplasty by using hydroxyapatite-coated femoral stem prosthesis （Corail）. Methods The curative effect and follow-up information of 141 patients （164 hips） with total hip arthroplasty by using Corail prosthesis during April 2007 and November 2010 were observed. Results A total of 107 patients were followed up, and the average time was 47 months. During peroperative period, 6 patients had fracture via femoral calear, who were healed after corresponding treatment ; 2 patients had postoperative infection, by joint retention 1 with stenoplastic treatment and the other with debridement in early period, and there was no recurrence with 2 years of follow-up. Eikonic examination showed that there were no lossening or subsidence of prosthesis, periprosthetic fracture, late infection, thigh pain or dearticulation. The peri-prosthetic high density of spot film area appeared in 15 hips. Harris score was significantly improved at the end of follow-up. Conclusion Corail stem prosthesis has advantages of easy operation, high safety, stability for long term and popularization. Effectiveness for long term is based on the characteristic understanding and appropriate implantation technique.
