摘要
目的:评价肝动脉化疗栓塞(TACE)联合体部伽玛刀治疗原发性肝癌的临床疗效及毒副反应。方法:105例肝癌患者随机分为2组:介入+伽玛刀组(综合组)52例,介入治疗组(对照组)53例。比较两组治疗后的近期疗效、临床受益疗效、生存期及毒副反应。结果:综合组、对照组有效率分别为92.3%和67.9%,综合治疗组的临床获益疗效明显较对照组高,两组1年生存率分别为88.5%(46),71.7%(38),3年生存率分别为40.4%(21),28.3%(15),1年生存率有显著差异(χ2=4.61,0.01<P<0.05)。近期疗效也显著提高(P<0.05)。两组的副反应血红蛋白及血小板下降无明显差异,白细胞下降、肝功能损害、恶心、呕吐等综合治疗组发生几率均明显高于对照组。但经过对症治疗均能顺利完成治疗。结论:介入联合伽玛刀能显著提高原发性肝癌患者的近期疗效、生存质量及生存期。同时其白细胞下降、肝功能损害、恶心、呕吐等毒副反应也明显增加,但经过对症处理基本不影响治疗。
Objective ： To explore the clinical effect and toxicity of transcatheter hepatic arterial chemoembolization （TACE） combined with gamma knife treatment on patients of primary hepatic carcinoma. Methods：All 105 patients with hepatocellular carcinoma were divided into two groups randomly： Group A - TACE combined with gamma knife （ combined therapy group） 52 cases ; Group B - TACE only （ control group） 53 cases. Results： The effective rate of group A and group B were 92.3% and 67.9%. CBR in group A was much higher than group B. One year survival rate of the two groups were 88.5 % （46）and 71.7 % （38）. Three year survival rates of the two groups were 40.4% （46） and 28.3% （34）. The decrease of hemoglobin and platelet of the two groups were basically the same. The occurrence and platelet probability of leukocyte decrease, liver function damage, nausea, vomiting in group A were much higher than that in group B, but these symptoms would be cured after treatment. Conclusion：TACE combined with gamma knife can dramatically increase the short term effect, living quality and survival time of the patients of primary hepatic carcinoma. And the side effects such as leukocyte decrease, liver function damage, nausea, vomiting increased, but they dont influence the treatment after treatment.
