摘要
目的:观察清热燥湿健脾法对早期湿热困脾型2型糖尿病者胰岛素抵抗及脂联素(Adiponectin)和抵抗素(resistin)的影响。方法:将88例湿热困脾型2型糖尿病患者随机分为试验组和对照组各44例,试验组口服具有清热燥湿健脾作用的中药100 mL,早晚各1次,对照组口服二甲双胍0.25每日3次。治疗8周后观察两组服药前后体重、血糖、胰岛素水平及脂联素(Adiponectin)和抵抗素(Resistin)的动态改变。结果:试验组和对照组对各指标治疗后均有改善作用(P<0.01或P<0.05)。实验组Adiponectin水平均较治疗前升高,Resist水平均较治疗前下降,治疗前后存在统计学差异(P<0.05),而对照组对Resist、Adiponectin水平的影响无统计学意义,且两组治疗后差值比较无统计学意义。结论:清热燥湿健脾法在降糖的同时,患者的体重明显下降,其疗效与二甲双胍相似。且能使HOMA-IR水平、IAI的绝对值降低,Adiponectin水平升高,Resist水平下降,从而改善胰岛素抵抗。
Objective：To observe the effect of the method of clearing heat, drying dampness and strengthening spleen on insulin resistance, Adiponectin and resistin in type 2 diabetic patients with spleen disorder due to dampness and heat. Methods ：88 type 2 diabetic patients with spleen disorder due to dampness -heat syndrome were randomly divided into experimental group and control group, with 44 cases in each group. The experimental group was administered 100 mL Chinese medical decoction （with the effect of clearing heat ,drying dampness and strengthening spleen） twice a day. And meanwhile, the control group was administered metformin with 0.25 g,3 times a day. 8 weeks later, the dynamic changes of body weight, blood glucose, insulin, Adiponectin and resistin in both groups before and after treatment were observed. Results ：The indicators of both experimental group and control group were decreased after treatment （ P 〈 0.01 or P 〈 0. 05 ）. In the experimental group, Adiponectin level was increased compared with that before treatment, however, resistin level was reduced after treatment,and there was significant difference before and after treatment （ P 〈 0. 05 ）, while in the control group as for the changes of resist level and Adiponectin level, there was no significant difference. In addition, and the differences in both groups after treatment were not statistically significant. Conclusion： The method of clearing heat, drying dampness and strengthening spleen can not only lower blood glucose （ in type 2 diabetic patients） but also reduce the patients＇body weight,similar to the efficacy of metformin. And the method is able to reduce the level of HOMA -IR,the IAI absolute value,increase Adiponectin levels,and lower resistion levels, and thus it can relieve insulin resistance.
