呼吸内科病房多重耐药菌分布特点与耐药性分析 被引量:8

Characteristic analysis on the distribution and drug resistance of multi-drug resistant bacteria in department of Respiratory Medicine of Yiwu Central Hospital

摘要 目的分析义乌市中心医院呼吸内科2012年度多重耐药细菌的分布特点及常见抗菌药物耐药率。方法选取呼吸内科住院患者的临床资料,对多重耐药菌分布及耐药性进行回顾性分析。结果共分离出257株多重耐药菌,65.4%来源于慢性阻塞性肺疾病患者,其中年龄分布以70-79岁(68例,26.5%)、80-89岁的(116例,45.1%)所占比例最大,257株多重耐药菌中G+菌29株,占11.3%,主要是耐甲氧西林金葡菌(MRSA)和耐万古霉素肠球菌(VRE),G-菌228株,占88.7%,主要为耐碳青霉烯鲍曼不动杆菌、产ESBLs肺炎克雷伯菌和多重耐药铜绿假单胞菌,对常用抗生素耐药率也较高。结论呼吸内科病房多重耐药菌感染增多,尤其是老年人和合并基础疾病的患者较多,所以严密监测菌株的分布,监测抗菌药物耐药动态,促进合理使用抗菌药物已刻不容缓。 Objective To analyze the distribution characteristics and drug resistance of muhi-resistant bacteria in department of respiratory medicine in 2012. Methods The clinical data of the patients in the respiratory department were chosen, and the distribution of the multi-drug resistant strains and the drug resistance were retrospectively analyzed, Results 257 strains of multiple drug-resistant bac- teria were isolated, 65.4% of which came from patients with chronic obstructive pulmonary disease. Hight incidence was seen in patients at ages between 70 -79 (68 cases, 26.5% ) and 80 - 89 ( 116 cases, 45.1% ) years. Among the 257 multi-drug resistant strains, there were 29 G+ bacteria strains (11.3%), with meticillin resistant Staphylococcus aureus (MRSA) and vancomycin resistant enterococcus (VRE) being predominant. There were 228 G - bacteria strains (88.7%), with carbapenem-resistance Acinetobacter baumannii, ESBLs Klebsiella pneumonia and multiple drug resistant Pseudomonas aeruginosa being predominant, and the antibiotic-resistance rate to most an- tibiotic was high. Conclusion The infection of multi-resistant bacteria is increasing in respiratory medicine department, particularly in the elderly and the patients complicated with underlying diseases. It's important to closely monitor the distribution of bacterial strains and trends of drug resistance to improve the rational use of antimicrobial drugs.
