序贯应用辅助-控制通气和压力支持通气抢救危重肺心病呼吸衰竭疗效观察 被引量:1

Clinical Observation of Sequential Application of Assist Control Ventilation and Pressure Support Ventilation on the Patients with Severe Pulmonary Heart Disease Complicated with Respiratory Failure

摘要 目的:探讨序贯应用辅助-控制通气和压力支持通气抢救危重肺心病呼吸衰竭疗效。方法:选择287例危重肺心病合并呼吸衰竭患者,均给予序贯辅助-控制通气和压力支持通气。观察本组患者通气效果。结果:压力支持通气后2 h的PaCO2、PaO2、pH值分别与机械通气前比较,差异有统计学意义(P<0.05)。压力支持通气2 h后的PaCO2高于辅助-控制通气后,而pH值低于辅助-控制通气后(P<0.05);压力支持通气后2小时的气道峰压、心率低于辅助-控制通气后(P<0.05)。结论:辅助-控制通气能及时有效改善肺心病合并呼吸衰竭患者呼吸症状,而后给予压力支持通气更符合呼吸生理需要。 Objective:To explore the effect of sequential application of sequential application of assist control ventilation and pressure support ventilation on the patients with severe pulmonary heart disease complicated with respiratory failure. Methods :287 cases with severe pulmonary heart disease patients complicated with respiratory failure were selected and given sequential application of assist control ventilation and pressure support ventilation, the effect of ventilation was observed in the patients. Results: PaCO2, PaO2, pH values after 2 hours of pressure support ventilation compared with those before mechanical ventilation,the difference was statistically significant (P 〈0.05). the PaCO2 after 2 hours of pressure support ventilation is higher than that after assist control ventilation, and the pH value is lower than that after assist control ventilation (P 〈 0. 05 ) ; peak airway pressure and heart rate after 2 hours of pressure support ventilation were lower than those pressure support ventilation ( P 〈 0. 05 ). Conclusions: Assist control ventilation can effectively improve respiratory symptoms of the patients with pulmonary heart disease complicated with respiratory failure, and then the pressure support ventilation which was given meet the respiratory physiological needs.
