摘要
目的:评价有创和无创序贯机械通气治疗肺结核并严重呼吸衰竭的临床疗效。方法:对2009年1月至2011年12月间于我院就诊的53例肺结核并严重呼吸衰竭患者经口气管插管正压通气,肺部感染控制后,其中29例拔除气管插管后行无创正压通气,作为无创组,余24例以同步间歇指令通气加压力支持通气方式撤机,作为有创组,对两组治疗效果进行对比分析。结果:无创组患者平均机械通气时间显著短于有创组,3-5d后成功脱机率显著高于有创组,有效率显著高于有创组,P<0.01。结论:与有创机械通气治疗相比,应用无创序贯机械通气治疗肺结核并严重呼吸衰竭效果显著,可于临床推广应用。
Objective： To evaluate the clinical efficacy of invasive and non-invasive sequential mechanical ventilation for the treatment of tuberculosis with severe respiratory failure. Method： 53 cases of tuberculosis with severe respiratory failure patients admitted to our hospital from January 2009 to December 2011 with orotracheal intubation positive pressure ventilation, 29 cases among them were treated with non-invasive sequential mechanical ventilation method, set as the non-invasive group, the remaining 24 cases synchronized intermittent mandatory ventilation ＋ pressure support ventilation to weaning, set as the invasive group,did comparative analysis of two groups of treatment results. Result： The average duration of mechanical ven- tilation of non-invasive group patients was significantly shorter than the invasive group, the successful off- line rate after 3-5 days was significantly higher than the invasive group, the efficient rate was significantly higher than the invasive group, P〈0.01. Conclusion： Compared with the treatment of invasive mechanical ventilation, noninvasive sequential mechanical ventilation in the treatment of tuberculosis with severe respiratory failure has a siznificant effect, can be promoted and applied in the clinical.
