摘要
目的 :探讨扩张型心肌病 (DCM)的体液免疫学发病机制。方法 :借助免疫转印技术及流式细胞仪技术同步分析了 33例 DCM患者和 2 8例正常人的血清中抗心肌线粒体 ADP/ATP载体自身抗体和 Th细胞亚群。结果 :DCM组 Th2 细胞在外周血淋巴细胞中占 (0 .5 3± 0 .2 3) % ,对照组 Th2 细胞占 (0 .2 2± 0 .18) % ,两组比较差异有显著性 (P <0 .0 1) ;DCM组抗心肌线粒体 ADP/ATP载体抗体的检出率 (6 0 .6 1% )明显高于对照组(2 1.43% ) (P <0 .0 1) ;DCM抗心肌线粒体 ADP/ATP载体自身抗体 ,阳性组中 Th2 细胞在外周血淋巴细胞中占 (0 .6 7± 0 .31) % ,阴性组 Th2 细胞占 (0 .2 8± 0 .14) % ,两组比较差异有显著性 (P<0 .0 1)。结论 :DCM中 Th2细胞与抗心肌线粒体 ADP/ATP载体自身抗体的产生有关 ,而且 Th2 细胞和抗心肌线粒体 ADP/ATP载体自身抗体参与了 DCM的病理过程。体液免疫反应的异常在 DCM的发病机制中起着十分重要的作用。
Objective:To investigate the pathogenesis of humoral immunology of dilated cardiomyopathy (DCM).Method:The autoantibody against myocardial mitochondria ADP/ATP carrier and the Th cell subsets was examined by using the immunoblot analysis and the flow cytometry in 33 patients with DCM and 28 normal control subjects.Result:The percentage of Th 2 cells detected in patients with DCM〔( 0.53 ± 0.23 )%〕were significantly higher than those in controls〔( 0.22 ± 0.18 )%〕,P< 0.01 .The percentage of the autoantibody against myocardial mitochondria ADP/ATP carrier in patients with DCM ( 60.61 %)were significantly higher than those in controls ( 21.43 %),P< 0.01 .The percentage of Th 2 cells in DCM patients with autoantibody against myocardial mitochondria ADP/ATP carrier positive ( 0.67 ± 0.31 )% were significantly higher than those with negative( 0.28 ± 0.14 )%, P< 0.01 .Conclusion:Th 2 cells is related to the production of autoantibody against myocardial mitochondria ADP/ATP carrier in DCM,and Th 2 cells and the autoantibody against myocardial mitochondria ADP/ATP carrier takes part in the pathogenesis of DCM.Our results further verify that abnormality of humoral immunity plays an important role in the pathogenesis of DCM.
